How Physiotherapy Helps To Manage Arthritis?

Arthritis is inflammation of one or more of your joints. Pain, swelling, and stiffness are the primary symptoms of arthritis. Any joint in the body may be affected by the disease, but it is particularly common in the knee.

Knee arthritis can make it hard to do many everyday activities, such as walking or climbing stairs. It is a major cause of lost work time and a serious disability for many people. There are 2 types of arthritis the knee joint in the human body can get afflicted with. They are:

Osteoarthritis

The form of arthritis which, with increasing pain, slowly wears down the joint cartilages is called osteoarthritis. This form of arthritis usually affects people after the age 40. The symptoms of osteoarthritis include:

  1. Severe pain in the knee joints
  2. Pain after walking up the stairs and it subsiding once you are on rest
  3. Severe pain after the movement of joints for a long time
  4. Pain that becomes worse in rainy days
  5. Joints becoming stiff after waking up in the morning but they improve in the latter part of the day
  6. Pain which also occurs in the thighs and the genital regions coupled with joints swelling and joints getting stiff after rest.

Rheumatoid arthritis

Rheumatoid arthritis is a chronic form of arthritis caused because of the knee joint inflammations. This form of arthritis can occur at any age. Being an auto-immune disease, its symptoms include, but are not limited to:

  1. Severe pain in the morning
  2. Mild fever accompanying the pain
  3. Joints suddenly becoming swollen, red and warm causing, immense pain
  4. Sudden stiffness of the joints
  5. Pain that increases in cold weather
  6. Mild fever, extreme tiredness and weakening of the muscles

Doctors are still doubtful about what exactly causes the disease; but the deformation of the immune system might cause the damage of the joints, causing people who are already suffering from obesity, smokers, and women, in general, more prone to this disease.

When the knee pain is diagnosed as a form of arthritis, the following treatments are suggested:

  1. If you are overweight or obese, losing some of those extra pounds can go a long way in reducing the pain.
  2. Muscle-stretching exercises are effective in keeping the knee joints flexible.
  3. Dry needling, cupping, taping can help to control the pain and keep the joint supported.
  4. Prescribed dosage of anti-inflammatory drugs such as Tylenol, Motrin, and Advil or injections of hyaluronic acid might relieve your pain.

If regular treatments do not work, you might opt for knee-replacement surgery and osteotomy (the process of cutting a bone with the help of surgery) which might better the alignment of the knee by transforming the bone shapes.

Physiotherapy for knee arthritis

Physiotherapy treatment is aimed at improving the symptoms of the disease (i.e. knee pain, swelling, stiffness), and you should begin to notice a positive difference within one or a few physiotherapy sessions. The main goals of physiotherapy for your knee arthritis are:

  1. Reduce your knee pain and inflammation.
  2. Normalise your knee joint range of motion.
  3. Strengthen your knee: esp quadriceps (esp VMO) and hamstrings.
  4. Strengthen your lower limb: calves, hip and pelvis muscles.
  5. Improve your patellofemoral (knee cap) alignment and function.
  6. Normalise your muscle lengths.
  7. Improve your proprioception, agility and balance.
  8. Improve your technique and function eg walking, squatting. If you wish to discuss about any specific problem, you can consult a Physiotherapist.

Hello, Thanks for the query. Since she is on treatment for hypothyroidism, it is important to check TSH level periodically. Because if TSH is not in the expected range (2 to 3 mU/L), then most of the symptoms come back. So please check that first. If the level is higher than mentioned, then increase the dose of thyroxine. Presence of swelling shows hypothyroidism is not in control. Thanks.

What To Expect Before Spine Surgery?

MS - Orthopaedics, MCH, Membership of the Royal College of Surgeons (MRCS), MBBS, FJR
Orthopedist, Delhi
What To Expect Before Spine Surgery?

Spinal surgery becomes inevitable when back pain cannot be managed with medications and/or exercise. This is a major decision and requires planning for many things before, during, and after the surgery.

Before a spinal surgery- This preparation will help in smooth recovery, especially if you do not have a full-time caretaker.

  1. Discontinue pain killers: Discontinue pain killers at least 10 to 14 days before surgery. These are blood thinners and can prolong bleeding during the surgery.

  2. Prepare for blood loss: Most people experience some blood loss, but not excessive.

  3. Use a toilet seat raiser: Sitting and getting up from the toilet seat may be difficult. The seat raiser is used to increase the height, making this movement easy.

  4. Enable easy access to common items: Before heading for the surgery, keep things which are commonly used within easy reach. This will help reduce movement and avoid searching (especially if someone else is going to be doing it).

  5. Stock it up: Cooking may not be feasible during the initial postop period, and so it is advisable to stock up food items (ready to eats, fruits, soups, etc.) which will come in handy.

  6. Slip-ons: Bending down and tying shoes may not be easy, so slip-ons can be used.

  7. Caregiving: It is always advisable to have someone stay over with you during the initial postop days. They could help with regular household chores, cooking, etc.

  8. Lifestyle changes: Ensure you eat well in the days before surgery, quit smoking, quit/use moderate amounts of alcohol, and exercise as advised by your surgeon.

After Surgery- Post surgery, there will be some pain and limitation of movement. It is important to understand that adherence to post-op instructions will improve the success rate of the surgery. Also, recovery time for spine surgery is slightly longer and affects overall quality of life, so psychological preparation is required.

  1. Postoperative medications: These will be given to control infection and pain in the immediate postop period, and should be taken without fail

  2. Rehab: The surgeon will recommend physical therapy and rehab exercises which need to be followed. Complete recovery may take anywhere between 3 to 12 months. During this time, care should be taken to avoid repeat injury.

  3. Support: Adequate back support should be provided using lumbar support and ergonomic chairs, and the right posture should be maintained. Ensure there is no undue strain on the back muscles.

  4. Weight management: With excess weight, there is too much strain on the lower disks. Therefore, weight should be managed to reduce this strain.

  5. Smoking and alcohol should be completely stopped, as healing can be hampered.

With some preparation, spinal surgery can be sailed through smoothly.

Paralysis & Stroke - How Physiotherapy Can Help?

Paralysis & Stroke - How Physiotherapy Can Help?

A stroke or paralysis can occur when the blood supply to that part of your brain is curtailed or drastically reduced. Due to this, the brain is deprived of the essential oxygen and the nutrients, and within a few minutes the brain cells can undergo severe damage and die. It is often treated as a medical emergency and can cause complications for life if left unattended.

Symptoms:
With regards to a stroke, a quick and an immediate treatment can be the difference between life and death. It is very much mandatory to note the symptoms of stroke and to impart the treatment accordingly. Some of them can include:

  1. Trouble with speaking: Your speech pattern gets affected, and you may have trouble talking.
  2. Paralysis: When the oxygen is cut off, a part of the body can become numb thus rendering it lifeless.
  3. Headache: A severe headache may occur that can make the body go into spasm. Dizziness and unconsciousness can follow it.
  4. Walking and mobility: You would be unable to walk on your own, or there can be a huge difference in your walk pattern. Most of the times, you might not be having any control over your legs.

Risk Factors
There are many risk factors associated with stroke and paralysis. The most common one is undoubtedly overweight. Physical inactivity, alcohol abuse, and drug abuse can contribute equally to the occurrence of stroke. Cholesterol levels, high blood pressure and old age may also be risk factors. Paralysis can be the biggest drawback of stroke where one part of your body can be lost to movements. You might also lose control of your face or arm, and this can affect your walking steps. This can be a life changing factor and more than medication sometimes physical therapy heals the person to an extent.

Physiotherapy for Paralysis:
You can feel helpless and frustrated, and this can lead to depression. There are many ways by which physiotherapy can help you. The therapies are made in such a way that you can try to lead a normal life without any hindrance, and this depends on the nature of the paralysis you are undergoing. In most of the cases, a person can get back at least 60% to 70% of his/her mobility back though not completely. Also, current medicines have advanced that there is a possibility to get back the movement in the paralyzed areas.

One of the major challenges you will face is to relax as the physical feeling of helplessness can hamper your thoughts in the long run. Stay relaxed and always remember that you are not alone. Take the help of a family member or a friend. Be relaxed all the time and make a determined effort with physiotherapy exercises. Avoid the lifestyle which you had before and embark on a healthier lifestyle and make sure you watch your diet at all times. Try to indulge in your favorite pass time and over a period start your daily chores without the help of an aid. You can undergo the physiotherapy exercises for life as they can be positive even after the condition subsidies.

6 Things to Know About Minimally Invasive Spine Surgery!

6 Things to Know About Minimally Invasive Spine Surgery!

The spine plays a very crucial role, both from a person’s movement and sensation point of view. It carries the nervous supply for a lot of internal organs and the lower body. By virtue of its structure, it also helps in movement through the disks. Due to various reasons, spinal injury is common, causing pain along the back, lower extremities, neck etc.

Medicines and exercise are the first line of treatment for most spinal injuries. The issue, however, is that these only provide relief from the pain and the medicines, but do not actually ‘rectify’ the underlying problem. Whether it is a herniated disk or a pinched nerve, the medicines and exercise can relieve the symptoms, but the disk continues to be herniated and the nerve continues to be pinched. Definite treatment is in the form of surgery, and with recent advancements in the field of spinal surgery, there are minimally invasive surgical options, which provide complete cure with minimal recovery time. Some of the benefits of this are listed below-

  1. Smaller incision compared to traditional surgical procedures, where only the injured area is accessed

  2. The surrounding tissues are untouched, unless they are injured and require repair

  3. Lesser bleeding compared to earlier techniques

  4. Lesser painful procedure

  5. Reduced hospitalization period

  6. Quicker recovery and return to normal activities

The question, however, is whether this is indicated in everybody. A detailed discussion with your surgeon keeping the below points in mind will help identify the answer for this. The main driver should be the answer to the question – will the surgery be able to effectively relieve you of the pain and symptoms? Additionally, the following factors help in getting better results out of a minimally invasive spinal surgery.

  1. Presence of symptoms relating to nerve compression, including pain from the spine down the leg.

  2. Recent onset of symptoms, with pain starting days to months before seeing the doctor. This indicates the condition is relatively new and so damage is minimal, indicating favorable outcome. Chronic pain conditions take slightly longer time compared to recent injuries.

  3. People with active lifestyle who engage in regular physical exercise are likely to have better benefits from the minimally invasive procedure.

  4. Smoking reduces blood flow to the spine, leading to disk degeneration and weakening of bones. Healing is highly slowed down and recovery is overall delayed.

  5. Excessive weight puts strain on the back, and so healing can be delayed or badly affected with more complications. It is not a contraindication, but be prepared for a longer recovery period.

Talk to your doctor keeping these points in mind, and you can bid farewell to your back pain forever. If you wish to discuss about any specific problem, you can consult a Neurosurgeon.

Greetings, amRaja of age 25. Came up with a humerus shaft fracture (transverse) before 6 weeks, bone seems to be in alignment and shows signs of healing .how long should I be in the cast and how to identify that it's about time to remove my cast and start with pt. Can see a fluffy cloud pattern around the fracture but inside the unity isn't 100 percent yet. Once healing Is complete, can I resume lifting weights slowly by increasing the weight?

MCh Ortho, ATLS (AIIMS), Diploma In Orthopaedics (D. Ortho), MBBS, MS - Orthopaedics
Orthopedist, Kolkata
Usually the cast is removed in 6 weeks as keeping it longer will cause shoulder and elbow stiffness. The fracture can take 6 to 8 months to heal. If the fracture has not been fixed with a plate or nail, lifting weight is not advisable for another 3 months. If the fracture has been fixed you can immediately start physiotherapy and lift weight upto 2 kg.

Hi lybrate-user If you cannot take calcium tablets you can take calcium naturally but before that, you need to have sufficient amount of Vitamin D levels in your body so that the calcium will naturally be absorbed. The recommended levels of vitamin D are 20 nanograms/milliliter to 50 ng/mL. The diet you need to take for joint pains and cramps is as follows: Spinach Bendi Soybeans White beans Foods that are calcium-fortified, such as some orange juice, oatmeal Vitamin D rich foods Foods fortified with vitamin D, like some dairy products, orange juice, soy milk, and cereals Cheese Egg yolks If you do not have nonveg you can take supplementary homeopathic Vitamin D3 (Filcal D3) one tablet once daily on empty stomach.

Hi lybrate-user, what you are describing sounds like an anxiety attack and since you have already been diagnosed with anxiety I suggest that in addition to medication, you should get to the root of the issue. Try some pranayam and yoga. I would also advise that you seek help from a professional counsellor. Hope you feel better soon.

Normally there has to be gap in the disk then only it will act as a cushion. If the space is reduced then there is a problem. He needs rest, physiotherapy and exercises. If the pain radiates to the leg then he needs Mri scan.

It seems that you are suffering with muscle stiffness. Generaly is can be cured with exercise and some stretching. But it can be symptom of some other major disease if it is associated with some other symptoms too. Do you have other symptoms like fever, redness, pain in left leg. As it is almost month you are having this pain my advice is you should go for proper and detailed consultation which includes detailed case taking, proper medicines, stretching and exercise guidance. But for once you can take Rhus tox 30 /3 dose per week.
