Are you taking any medication for hypothyroidism because normal TSH is necessary to stop weight gain and to reduce the excess weight you have to take homoeopathic treatment.

According to some professional doctors, if I am not pre diabetic patient, Fbs show show 70 to 110, for me it will show 114, 112& some time it show 97 and PPbs normal showed be 80 to 140, mines it shows 135, 138 & 125 like that, but HbA1c - 5.6,really I don't have pre diabetes? Bcoz of this tension I am suffering from headache, not only me becoz of my behaviour my kids are very sad, even my husband is fully tension. Now what had happened means every intake in take after that I want test the reading, it has become habit since from one week. I am became nervous due to this change in my body. Bhuvanshreeji, Thanks for the query. I have seen the details provided I really do not see the reason for you to be worried. I am giving below the criteria to diagnose various stages related to blood glucose. 1)Normal: FBG: 70 to 110 mg, PP <140 mg & HBA1c%<5.9. 2) Pre-diabetes : FBG 108 to 125 mg, PP 141 to 199 mg & HbA1c% 5.9 to 6,4. 3)Diabetes : FBG 126 mg and above, PP 200 mg and above & HbA1c%6.5 and above. Therefore there is really no reason to worry. Some of FBG levels have shown 112, 114 etc. That could be called as pre-diabetes. But PP levels are absolutely fine. Please remember Type 2 diabetes is a preventable condition, if one maintains good healthy diet, regular exercise of over 1 hour a day and maintains normal weight. Your BMI 24.21 kgs/M2 (normal <23 kgs/M2), which is marginally high. You can cut down a few (3 to 4 kgs) weight very easily. Madam please understand a person with diabetes can live an absolutely normal life if the blood glucose is well controlled. At present level you are neither in prediabetic state or any where near it. Plus your weight is fairly good. So if you take care of diet, take healthy balance diet, eat more of green leafy vegetables and fruits, plus avoid junk foods and pastries and bakery items and exercise for 1 hour a day(brisk walking), then you will be fit and free from diabetes. So please do not worry and spoil the pleasant atmosphere at your home. I am sure you do it. Thanks

Hi! Good evening sir / madam suffering from thyroid problem this week I tested TSH value 7.70 so which tablet I have to use? How much mg tablet?

MBBS, CCEBDM, Diploma in Diabetology

Mr Mohiuddin, Thanks for the query. I think this question is with regard to your wife, if so you should have provided her height and weight. That would have given some idea as to whether she is overweight or obese. Giving your details has no meaning. TSH 7.7 mU/L is above normal, but falls in the subclinical stage. That normally is not treated unless there are specific symptoms related to hypothyroidism. Thanks.

Mr Rakesh, Thanks for the query. Cycling is a good exercise. But should be carried out for a substantial amount of time like an hour or so. Whether one measures kilo meters or minutes does not matter. It is a good exercise for everyone including diabetic person. Thanks

Diabetic Nephropathy - Tips To Help You Manage It!

MBBS, MRCGP ( UK), Diploma in Diabetes (UK), DFSRH (UK), DRCOG (UK)
Endocrinologist, Hyderabad
Diabetic Nephropathy - Tips To Help You Manage It!

Diabetes is not an isolated disease but a syndrome. It brings with it a host of issues, affecting almost all organs, and that is the reason it is a dreaded issue. Diabetes is the result of chronic inflammation and this inflammation gradually spreads to all other organs. One of the main organs adversely affected is the kidneys. The kidneys are essential filtration agents, and impaired kidney function can have a series of bad effects on the body.

The good news is that not everybody with diabetes will have diabetic nephropathy. However, risk factors for developing diabetic nephropathy are:

  1. Severe, uncontrolled diabetes
  2. Family history
  3. Prior kidney disease, even chronic infection
  4. Race or ethnicity (native Americans, African Americans, etc.)
  5. Preexisting hypertension or high cholesterol levels
  6. Obesity

If you have these risk factors, then all efforts should be aimed at keeping blood sugar at highly optimal level. Once it reaches beyond a certain level, it is very difficult to get it under control.

Symptoms

There would be no symptoms in the initial stages

  1. If you suspect that increased sugar levels could be damaging the kidneys, it is imperative to do renal functions annually to test for onset of kidney malfunction
  2. There could be swelling of the feet and ankles
  3. Urine tests may indicate increased levels of protein in the body (serum albumin in particular)

Management

The idea is to arrest or stop damage to the kidneys.

  1. Reduce and maintain blood pressure within the recommended levels. This will depend on the age and overall health status. Using angiotensin converting enzymes or angiotensin receptor blockers is the most effective way to control hypertension and related kidney damage
  2. Keeping blood sugar within the recommended levels helps reduce or stop damage to the blood vessels in the kidneys and thereby, prevent progression
  3. Maintain optimal levels of cholesterol, as this only adds to hypertension, which further worsens kidney function.
  4. Eat a heart-healthy diet and exercise regularly. It is a vicious cycle with components including hypertension, high sugar levels, increased heart disease, which all only continues in a circle nonstop. Breaking this cyclical trend is very important.
  5. Diet should not be too rich in protein, as the kidney function is impaired and may not be able to handle too much protein. Talk to a nutritionist as to how much protein is required and follow it strictly.
  6. Reduce salt levels in the diet, and strictly do not add extra salt onto your plate
  7. If you are a smoker, this is a great reason to quit, as it affects all the above conditions.
  8. Maintain weight under optimal levels.

If you wish to discuss about any specific problem, you can consult an endocrinologist..

Archanaji, Thanks for the query. If one is on treatment for hypothyroidism and if the thyroxine dose is adequate, then TSH should be maintained between 3 to 4 mU/L to be free from any symptoms in a non pregnant woman. However, if conception is desired then TSH should be maintained <2 mU/L. Later during first trimester it should be 0.1 to 2.5 mU/L, 2nd trimester :0.3 to 3 U/L & 3rd trimester 0.3 to 3.1 mU/L. So present dose has to be increased by 25 mcg per day. Then check TSH after 6 weeks to see if desired level is reached. Thanks.
