This appears like Ear drum perforation as a result of cold and middle ear infection. Ear must be kept dry with no liquid entry of any type. Wipe outside with cotton buds. Rebound nasal obstruction is side effect of any xylometazoline drop like otrivin.
Stop using this drop.
Instead you can use saline nasal spray.
