For vitamin d levels as low as 4, I would suggest arachitol injection of 6 lac units taken once every 15 days for 3 cycles. Follow up with the satchet later on. Also take diet consisting of ragi, fruits, milk and milk products. Add some regular exercise.

yes. Levels are low. Initially you have to start with injection or medications till levels normalize. Then with the healthy eating habits you have to maintain it.
Sources of Vit D best is early morning sun rays
.cod liver oil. Fish. Eggs. N some mushrooms. Fortified cereals n yoghurt. Tofu.etc.
