Anxiety and depression oftentimes go together. The weight gain is not surprising some due to the medication, and some due to lower metabolic rate, and low exercise, and emotional eating. There may be genetic influences for diabetes and she must take precautions. There seem to be some psychological problems that need investigation and treatment from a counselor too. I believe there may be some genetic predisposition or there has been some childhood issue, related to anger and fear, that needs urg...more

I'm sorry to hear about the difficulties you're facing with your health. Managing multiple chronic conditions like dysthymia, anxiety, insomnia, chronic fatigue syndrome, and overactive bladder, along with the side effects of various medications, can be extremely challenging. If you have been on antidepressants for 4 years continuously and for the previous times you have had four different episodes it seems like you have depression with dysthymia. We call it double depression and it needs treatment on a long term basis. You also mention that you have had ect. However you do not mention how many treatments you received and what was their outcome. If you want to go off of antidepressants you might need to gradually taper them off, while being on a different kind of the...more

Dear lybrate-user,
You need a proper diagnosis before going in for treatment. If h pylori is confirmed, it needs active treatment. Premature ejaculation and loss of sexual desire also needs to be diagnosed and treated. At Japan, you were treated with Lorazepam (available in India). Instead of brotizolam, you can try etizolam available in India. Trazodone available in India. Suvorexant is another mild hypnotic. You don't need so many hypnotic and anxiolytic. Lorazepam or etizolam will do. Esc...more

Hi, by the list of medications I can understand that you have tried a lot of medicines. Still your problem is not resolved. Since you are near to bangalore the best way I could suggest is visit nimhans bangalore and consult a psychiatrist and a therapist for your current problem you might need more of therapy and a different kind of antipsychotic which works for dysthymia. The medicines which you have mentioned are antidepressants and does not necessarily work in dysthymia.

Dear Sara,
Your doctor at Pakistan is equally good and she/he has been trying good treatments like antidepressants, mood stabilisers and ECT. Please continue with her/him. Telemedicine in India has jurisdiction in the same country only. It is difficult for you to undergo treatment in India because certain basic tablets only can be prescribed through telemedicine. Intractable depression needs lot of experimental tablets as add on.

Dear lybrate-user,
Most antidepressants work more or less at the same level. Mostly, you go by the side-effects - whether they are acceptable by you. Both Vilazodone and Bupropion have minimal sexual side effect. Fluoxetine has more erectile dysfunction.
Weight and sedation are also to be taken into account. If you can tolerate effective dose of TCA, you can try. Otherwise go for Venlafaxine. The only tip is - it is not the antidepressant. It is the duration (minimum 3-6 weeks) and dose ...more
