Dear Since your diabetes is of long duration it seems your capacity to make insulin is exhausting. It is best to control your sugar by changing insulin. Lantus is basal insulin so you have to take insulin with each meal too initially. Once good control is achieved we may try twice daily regimen. I would rather change his medications to a long acting gliclazide metformin combating together with a Dpp4 inhibitor also. The reason is a poor HBAIC of 7.5 with the present drug .diet and regular excercise goes without saying in all diabetics and a dietician advise is recommmended.
