Dear Mr. lybrate-user, I note that your fasting blood sugar is under better control after you changed your medication. However your post meal blood sugar is on the high side and I note that your weight is only 60 kg which is a bit low for your height. Check with your family doctor about the need to take insulin for improving diabetes control and to gain weight to optimal body weight

Dear Doctor, I am 73 years old (DOB 30/03/1941) male. Recently, I developed pain in my Right Knee. I went to the Orthopaedic Surgeon who had carried our orthoscopic about twenty two years ago. He had the removed bits of cartilage and since then I had no problem. This time he saw my X-rays (copy attached) and said that I needed replacement surgery ? first to the right knee and then to the left knee. On request he prescribed Tab Nucoxia 90 and Vitamin E 400mg. I am diabetic (2) and have high blood pressure, both under control with medication. A pacemaker was implanted on 01/06/2012. It is working fine as per my last check up on 03/11/2014 (next check-up due on 3/5/2013). I am having following medicines ? Tab NatrilAm, Tablet Telma (80), Ecosprin (75), Urimax (0.04), Metosartan (1000) BD, Trajenta (5), Amaryl (2), vitamins and calcium). My request is to advise:- 1. Is there an alternative to surgery? Is Joint replacement surgery a must? 2. Is Tab Nucoxia 90 safe with the medicines I am taking, and how long should I continue?

Total knee replacement is last resort for such cases. This procedure is very much effective and safe and results are very good.
We do tkr frequently and patient is able to walk 2-3 day with ability to cross leg. Diabetes and hypertension are not to worry for this surgery

Thank you Dr. insulin taken on 12-7-20: novomix 30/70--12 units before breakfast. 13 units novomix and before dinner13 units novomix. The results are: 13-7-20: fasting 136. Ppafter breakfast: 225…. Ppafter dinner: 239. Earlier my local Dr. put my on novomix 30/70 twice a day. Morning 22 and before dinner with 12 units. Butpps after breakfast and after lunch are around 225 to 250. After that local Dr. changed to 3 times novomix 30/70 with regimen 14,--14--13. Still pps after i and lunch are more than 225 only. My questions are :1) you stated novomix morning and before dinner with lantus. But this lantus is not taking care for 24 hours. My experience with lantus which is taken at night is working upto fasting level only. Due to my above experience, small doses of novomix after breakfast and dinner, will take care my high pp levels. For afternoon, you have not stated any insulin. Kindly review my case particularly keeping into account my high pp levels. Of course, if I take 3 times novorapid and onetime like tresiba, my levels may be alright. But 4 times punching my body with pain and often injections. 2) local Dr. stopped my gliptins like janumet or linagliptin when I stated losing of my muscle strength at thies area. As you stated, my injection area in abdomen or thies area not affected. If you say confidently, I can start any other medicine suitable to me if you suggest but my muscles should not be reduced. I am following strcy diet control with 1600 kilo cal. 3 months back my hba 1c is 7.4%. Pl advice and help me Dr. Regards prabhakara rao v vijayawada age 63 weight 73 kg.

I note that the problem is with the pp values. Even on the present regimen, your hb a1c is reasonable particularly if you have had diabetes for > 10 years. I would suggest you try the follwoinf dietary changes:
if your fasting values are > 120 and pp values go above 180, reduce the quantity of carbohydrate (rice, wheat, rava, maida) in each meal and increase the quantity of protein (dhal, egg white, fish or chicken. Monitor blood sugars with glucometer as follo...more

My wife (48 yrs) was suffering from diabetes 2 for 25 years. She got severe pain on the hip during July 2018 and nephrologist diagnosed as kidney problems. Prescribed steroid & other tablets and relieved from pain. Now past 2 months steroid medicines were stopped and following tablets are given. Albunil (1-0-1), Prolomet-xl 50 mg (1-0-0), Zyloric or Uricare 100 mg (0-0-1), Sodamint 300 mg (2-2-2). Endocrinologist due to kidney problems stopped sugar control medicines immediately. Then after treatment from Nephrologist, our Doctor Endocrinologist prescribes following due to uncontrollable sugar level for the past 3 months (1st month tablets only, 2nd month along with Mixtard injection and 3rd month, from 23/10/18 onwards, Mixtard with Actrapid injection: Mixtard Inj 30/70 40 IU) 0-0-12, Actrapid Inj (10-10-10), Trajenta 5 mg (1-0-0), Amlodac 5 mg (1-0-0), Thyrox 25 mg (1-0-0).Before (23/10/18) taking above treatment, Fasting Sugar level is 263. HbA1c is 12.7.Serum creatinine is 2.2 Urea is 68 Uric acid is 6.2 Urine Protein spot is 134, Urine protein and Creatinine ratio is 1.25, Creatinine spot is 106.94 BP level is 150/80 Pulse 92 / mt Weight is 59.7 She was taking walking exercise regularly for 20 years. Now she feels tiredness to walk and stopped walking for past 3 months. Please advise what steps will have to be taken for to bring her back normal state.

She is having diabetes which is uncontrolled with diabetic nephropathy. You should follow advice of Endocrinologist and Nephrologist strictly. Encourage the patient to be positive and physically active.
