It is better you get a test called as Hba1c done. It will give an idea of the average sugar level in your body over past 3 months. It will help to decide whether you need a dose increment/change of medicines.

From the glucose levels mentioned it is clear that after food rise is quite significant. Ideally fasting glucose should be < 100 mg, PP 160 1to 180 mg (considering her age) & HbA1c% < 7. (not mentioned). From the details it is clear that she has uncontrolled diabetes. It needs up gradation of current treatment, plus there is a need for closer look at her diet intake and daily physical exercise. Get your c-peptide test done after 8 hour fasting. C-peptode is a amino acid complex released when a molecule of proinsilin breaks down to insulin hence it is an indirect measurement of insilin. Normalrange is 0.5-2 ng/ml of blood. If your c-peptide is less than normal means insulin is less and indicates type 1 diabetes, if more than more insulin is produced as in type 2 diabetes.

Your ACR seems fine. Please mention your serum creatinine (mg/100 ml) and urea concentration to calculate eGFR. Also mention the fasting, PP glucose levels and HbA1c%. Plus give following details : dietary pattern, extent of daily exercise being done, lipid profile, weight and height. On going through all the parameters I will be able to give specific comments. The dosage of various medicines for glucose control can be adjusted on the basis of patients needs, doctor's advise and also based on dietary intake. Usually Glimepiride is either given once a day or twice in some cases. Metformin can be given 3 times a day again depending upon the need. Voglibose is usually recommended with each meal, that is three times a day. As it is mainly for reducing post food glucose rise. Thanks.

Madam, I have seen the query. Your HbA1c% is very high. Similarly fasting & PP glucose also are quite high. Isaka matalab aap ka diabetes bilkul hi control me nahi hai.
