The drug you mentioned is a very good drug for hypertension in a diabetes pt. Moreover it has reno-protective effect. Having said that, you need to monitor serum creatinine and serum potassium if you continue it.

Yes mam you bp demands the start of telma at 49 mg.
Ideally you need dual combinations therapy and the uptitration or downgradation I as per your bp response also try to control your risk factor for bp which you have not mentioned.
Would like to know how your bp stays in 7 days so that I can suggest some modifications if any required.
