I have seen the details given. However, aprt from mentioning presence of leukcocyte esterase in urine, there are no relavant details like Fasting, PP glucose levels, HbA1c%, TSH levels, BP readings, height, extent of daily exercise, presence of any symptoms related to infection and lipid profile. Enlarged inferior turbinates are one of the causes for blocking of the nose. A steroid nasal spray helps to a certain extent but the mechanical problem has to be rectified. Turbinectomy (trimming of the turbinates) will help by allowing more air flow inside the nostrils & the blocking will come down considerably. Medication will only work if you have an allergic component. Consult an ent surgeon who will diagnose & treat you accordingly.

Apply permethrin 5% (single application) to take care of residual infestation (if any). If you have applied permethrin last time then try ivermectin cream this time.
After that apply thin layer of fluticasone cream for 5-7 days.
Must take all the precautions & prevention explained by last treating physician for scabies and it's treatment.
