A 55-year-old builder attends for a clinical review. He has a history of gout and hypertension which are both well controlled with ramipril 5 mg once daily and allopurinol 100 mg once daily. He has no history of cardiovascular disease, but both his mother and a maternal uncle developed type 2 diabetes mellitus in their late 60s. He is physically active with his job and enjoys watching his favourite football team play on weekends. He admits that he often drinks about 5 or 6 pints of beer when he watches a big game and will also drink about 10 cans of beer at home during the course of the week. He has gradually put on weight over the last decade and currently weighs 95 kg, bmi 30 kg/m². His blood pressure is 135/85 mmhg. You organize a series of investigations: total cholesterol 7.0 mmol/l, hdl cholesterol 0.7 mmol/l, triglycerides 2.4 mmol/l, fasting glucose 6.2 mmol/l, hba1c 44 mmol/mol hb. Questions 1. What is the most likely reason for his low hdl cholesterol?

Hi i'm, 50 years old recently diagnosed with heart failure and been told I need double valve replacement n the future as I contracted rheumatic fever as a child and not known about it till now and it's damaged both my valves av got aortic stenosis and mitral regurgitation am on ramipril and furomeside water tablets all this started in july after having all the tests as in bloods ,x-rays, echocardiogram ,blood pressure was high and the symptoms I had and the tests they diagnosed this I was due to go in hospital yesterday for angiography at cardiac centre but was scared so didn't go as I am awake wen they do this procedure as in they numb my wrist and that's it i'm terrified and have got fluid n my lungs have in and off since july i've got moderate to severe valve mitral regurgitation and aortic stenosis and ive had left side under breast discomfort for last few hours think it's with fluid buildup but constantly feeling tired to do anything am scared to go and get checked out incase they say nothings wrong with me i'm under a cardiologist consultant for this illness but feel i've no one to turn to regarding this xx.

Hello sir/madam I am 46 years old male person 3 years back i was met major accident and also I was in coma 20 days now I am ok but few ago am feeling bad about my legs are swelling when I was sleep in night, morning its clear by evening same problem reflected, I have only hypertension. No diabetic now I am using amplodine 5mg mor, evng 2.5 ramiprill please tell whats problem, every test normal -lipid profile-lft etc.

In some individuals , amlodipine can cause swelling in the feet . Stop amlodipine and ramipril and start tab olmezest H 20mg once daily .. Hope the swelling goes . You should consult after one week of change in medications

My father who is 67 year old recently had a heart attack few days ago. He has a history of psoriasis. No history of diabetes or any other medical condition. According to the angioplasty results in the distal left main coronary artery there is 50% stenosis. There is type 3 and calcific vessel showing proximal 90 percent stenosis. Lcx,lima, renal are normal. The doctor has suggested him to go for bypass. However my father is apprehensive about bypass and want to explore other options like ayurvedic or chelation therapy treatment. Can anyone please suggest will these therapies help him in getting better without surgery? Is there any major risk involved if he opts out of surgery but makes significant lifestyle changes. Continues his medication and does some alternative therapies? Please help.

Though CABG is the best answer for this but if your father does not opt for it he can continue with high dose statin, aspirin,betablocker and ramipril in regular consultation with a cardiologist. He can do well.

One of my relative aged 60y suffering from hypertension. Her pulse rate also remains high 90-99. She is taking telmisartan 40 mg for many months. Another doc prescribed ramipril5+hydrochlorothiazide 12.5 mg for high blood pressure. Should she go with the same medicine & also tell me what precautions she will take please suggest immediately.

If already on telmisartan n cilnidipine'no need to change to ramipril n hydrochlorthiazide. For high pulse rate pt may req adding med after consultation. U can connect with me n then I shsll modify if need be.

If the BP is low only when you take alcohol you avoid alcohol and as it is normal at other times noting need to be done

When should I take ramistar 2.5 mg tablet everyday. I was treated for heart attack .I went through angiogram. Presently after 3 months l got normal ECG report with normal BP.

Shalini ji, Thanks for the query.
Madam the BP reading given is fairly high, suggesting there is a need for upgrading the treatment. There is no mention about TSH and Levothyroxine dosage as well as blood glucose levels, HbA1c%. So there is no way to suggest any thing. Thanks.
