My son 11 years old has bilateral hydro utero nephrosis (moderate dilation of pelivcalceal system and ureter is noted with mild urothelial thickening) and also has cystitis, with bladder wall thickening of 5.6 mm. He spotted blood drops at the end of urination. Bun/creatinine normal. Doctor gave antibiotic and all bacterial and uti symptoms are cleared, took mcug and report is normal. He also has constipation, both usg and ct reported fecal loaded large bowel loops. He is on continuous anti-biotic bactrim at night, suggested to do usg after 4 weeks. He had gross hematuria 2 years ago, that got cleared after 8 days course of sporidex and citralka. Now, my doubts are: 1) why he is getting repeated uti? 2) can anti-biotic alone cure hydro-utero nephrosis? Don't know if hydro nephrosis is due to current cystitis or previous uti 2 years ago. 3) is this treatable or not. Very much worried.

1) most common cause of repeated uti is a bad valve but mcug has ruled that out the next common cause could be residual infections and also ureteric stones and strictures predisposes to urinary tract infection. I would like to see all the reports of your child. This 33 years old female presented with c/o fever 1 week, breathless weakness, loss of appetite 1 week, bp 110/70mmhg, p/r 1116/min, temp normal, cvs s1+s2 normal, chest right low air entry, cns cons & oriented. Patient investigated & diagnosed as above managed conservatively by iv antibiotics (cefobran, ppi and other symptomatic treatment. Pleural tapping done on 25/02/2020 under la with aseptic precautions from pleural cavity & 1500 ml straw fluid aspirated. Post procedure uneventful. My att started on 29th feb 2020. I am taking same dose since 29th feb 2020. Kindly guide me about dosage whether I have to reduce or take same dosage. Medicine details: rifampicin isoniazid pyrazinamide and ethambutol hydrochloride tablets ip forecox each film coated tablet contains: rifampicin ip                               225 mg isoniazid ip                                  150 mg pyrazinamide ip                         750 mg ethambutol hydrochloride ip 400 mg dosage: 2 tablets per day omez d omeprazole pyridoxine hydrochloride tablets ip 40 mg benadon.

ATT is usually taken for 6 months. After 2 months, pyrazinamide is stopped and other medications should be continued.
In your case case, i suggest you to repeat a CXR to look for the response and amount of remaining fluid. Also continue rifampicin, isoniazid and ethambutol for atleast 3 more months. You can stop pyrazimamide.
