Asked for male, 32 years old from Bilaspur

Hello sir, My dad is 64 years old. He suffers from type 2 diabetes and hypertension. He takes olmezest am 40 for HTN. My question is - can he take OLMAX LN 40 as it contains another calcium channel blocker cilnidipine at 10 mg (5 mg cilnidipine is not available with olmesartan. As per my research on pubmed articles are in favor of cilnidipine as cilnidipine is an N-type and L-type CCB that also inhibits sympathomimetic activity in contrast to other DHPs and amlodipine therapy has adverse effects, most commonly pedal edema. So Cilnidipine being an N-type and L-type CCB is associated with lower incidence of pedal edema compared to the L-type channel blocked by amlodipine.

Clinically seen effect of Cilnidipine is different from Amlodipine in a lot of patients. Although, 10 mg of Cilnidipine is considered equivalent to 5 mg of Amlodipine, a shift from Amlodipine to Cilnidipine may lead to reduced efficacy atleast fir the first 1-2 weeks. You may change the medicines if unacceptable pedal edema is present.

but in some patient the olmesartan (GROUP ARB) may cause dry cough , so change and check response.
u may have asthma with hypertension

Hello!
Olmesartan is usually avoided in CKD for the fear of raising potassium levels. Take a second opinion.

Moderate High B.P since 2001 Severe high B.P since 2014. Took Amtas 2.5 mg up to 2008 then Amtas 5 mg till 2015. From 2015 started fluctuating B.P upto 160/100. Found 60% heart blockade in angiography creatinine increased from 0.8 to 1.4 and then in 2016 to 1.7. Now for last one years it has not increased. B.P is also 120-130/80-86 but with high medications. olmesartan 40 mg twice daily .telmisartan 40 mg with beta blocker 25 mg once daily. I am now having constipation since last six month. I take dulcolax alternate day. I used to smoke 30 cigarette a day and two 60 ml whisky. But for last 18 months stored totally. How to reduce medication.

Hello sir, My dad is 64 years old. He suffers from type 2 diabetes and hypertension. He takes Olmax LN 40 containing olmesartan 40 mg and cilnidipine 10 mg for HTN. He also takes an antiplatelet drug Clopilet A 75 (containing Aspirin 75 mg and Clopidogrel 75 mg) and a statin Lipigo 5 mg (containing rosuvastatin 5 mg). 1.Can he take omega 3 fatty acids like EPA and DHA 500 mg daily? Omega 3 fatty acids have been found to be very useful in reducing synthesis of prostaglandin series 2 molecules and thus are anti inflammatory. Thus is also good for cardiovascular and joint health. The American Heart Association (AHA) also recommends consumption of upto 1000 mg of EPA+DHA for heart health daily.

83 years old weight 70 kg. Diabetic with resistant blood pressure for over 20 years. Also have sleep apnea last 6 years. With treatment everything under good control. About a year and a half back suddenly my diabetes became normal ,had to stop my medication. This lasted for one month. This has happened two times more with a gap of 3-4 months. This time this started 2 1/2 months back and continuing. I have stopped my medication.in fact this time I am eating a lot of fruits and eats like dosa idli etc. To keep sugar normal. I am a vegetarian except that I take 2 egg whites. I have stage 3 ckd gfr 57 and avoid taking high phosphorus foods. For diabetes I am taking amree plus an ayurvedic medicine. For bp olmesartan 20 mg, cilnidipine 10 mg, moxonodine 0.3 mg, and indipimide 1.5 mg.

This is usual story of long lasting diabetes. As ckd advances blood sugar becomes normal or even low as most of the antidiabetic medicines are excreted through kidney and pt feels that his diabetes is cured and he does not need medicine.
Please take care of your ckd under supervision of a nephrologist.

I am 74 years. On a routine check up (the check up got hampered due to corona scare) the following noticed 1) hb%-12.8 g/dl (minm-13) 2) erythrocyte count-4.45 (minm-4.5 million/ cu. Mm) 3) lymphocyte-17.1%(minm-20%) 4) eosinophil more -6.6%(it should be 6 maxm) 5) pcv-38.8%(minm-40) what is your suggestion although no such urine complication so far. On a routine check up creatinine 1.41 mg/dl, urea 38 mg/dl. I have no diabetes. But blood pressure and cholesterol kept under control by allopathy medicine. Acute insomnia problem I have. Blood pressure and cholesterol under control by medication I take for for bp olmesartan 40 plus hydrochlorothiazide 12.5 for cholesterol atorvastatin 5 plus fenofibrate 72.5 whether any medicine is there to lower creatinine. Please advise.

Nothing is that critical but you need one md physician as creatinine just deranged, eoshinopil also just deranged.
They need regular follow up.

Avoid nsaids repeat creatinine after 2 week have adequate volume of water .not too much don't be so dry as well.
Go for urine routine micro and usg abdomen and kub
check electrolytes, phosphorus.
