Hi, My father-in-law was having fever from few days. After done with all reports, it was found that he is having some mass lump in his sinus. After getting that removed, he did the biopsy. This was the biopsy report -specimen biopsy left maxillary sinus clinical diagnosis? Fungal sinusitis gross description multiple soft tissues-0.3 to 0.5 cm, aggregate-1x1 cm. All in 1 block. Microscopic description section shows polypoidal sinus mucosa with many aseptate, broad happy with many aggregates of thin angled septate hyphae. Also seen are aggregates of pigmented spores and cannot like structures. Tissue invasion and necrosis is evident. No evidence of malignancy. Diagnosis biopsy left maxillary sinus- invasive mixed fungal infestation with fungi morphologically resembling mucormycosis, aspergillosis (fungal balls) and spores of pigmented fungi. Fungal culture correlation is required for exact subtyping of the various fungi. end of report all stained slides and paraffin blocks labeled h19/15568 returned along with report. Please preserve them carefully. Specimen will be discarded after one month of receiving of specimen. Our doctor suggested to go for amphotericin b injections for at least 21 days. My question is what is the dosage needs to be given and is it harmful, as he is diabetic? And how much gap is needed between 2 drips?

Amphotericin b for injection should be administered by slow intravenous infusion. An intravenous infusion should be given over a period of approximately 2 to 6 hours (depending on the dose) observing the usual precautions for intravenous therapy. The recommended concentration for intravenous infusion is 0.1 mg/ml (1 mg/10 ml).

Clavulanic acid is not effective if given alone hence we give it in combination with amoxicillin and hence the dose reduces because two drugs are acting at the same time.
