Hello lybrate-user. In addition to the psyllium husk, you may want to add a paraffin-based laxative which will help you pass your stools smoothly. Topical nifedipine/lignocaine is usually not contraindicated in pregnancy, however, I would suggest you consult a local surgeon for a proper examination and prescription regarding the same. Using some topical lignocaine 10-15 mins before stools and immediately after may keep your pain somewhat under control.

Hello sir please go through my article
What is an anal fissure? — An anal fissure is a tear in the lining of the anus, the opening where your bowel movements come out (figure 1). Anal fissures cause pain, especially during a bowel movement.
There is a muscle that wraps around the anus and holds it shut. It is called the "anal sphincter. The sphincter gets tense when the anus is injured. In people with anal fissures, the sphincter goes into spasms, which can lead to further injury.

Hello, thank you for informing me about your problem,
Constipation is a condition of the digestive system where an individual has hard feces that are difficult to expel. In most cases, this occurs because the colon has absorbed too much water from the food that is in the colon.
The slower the food moves through the digestive tract, the more water the colon will absorb from it. Consequently, the feces become dry and hard.
When this happens, emptying the bowels can become very painful....more
