Dear dortoc im simon and my age is 31, I has been admitted under medical ward before on 2019 pass two years, it was 1) diagnosed with cold aiha disease with megaloblastic anaemia tro pernicious anaemia and was transfused 3 pint pack cells and 2diagnosed with thyroid storm to hyperthyroidism during admission was given radioactive iodine therapy ,propranolol 40 mg and carbimazole 20 mg and cardio echo enlarged large ventricle, lvef 55%, enlarged left atrium, mild Mr. ,trivial tr with minimal pericardial effusion recently 11/2020 was repeated echo lv enlarged, thickened ivs, lvef 30% grade i, diaslic dysfunction tapse 13 mm rv normal size and function mv, tv,av pv normal no pericardial, pleural effusion seen and hyperthyroidism admission was given bisoprolol 2.5 mg and carbimazole 5 mg daily admission was plan to given 2nd time radioactive iodine therapy ,to control echo not getting worse during covid issue treatment has postpone to april 2021 currently stressful have started back symptom of thyroid storm trouble breathing, chest pain ,and leg swelling, what should I do please advise, is that possible my heart can recover? If no getting worse how long more I can lives?

Hi, patient has vision problems since last 1 month now. She has had problems in the past like breast cancer 7 year back. Only chemo was taken, no radiation. One breast removed. 2 years back she developed *hyperthyroidism* and she also had to put one stent. 2 months back she was hospitalized for back pain and mri was done and she had some *l4l5l6* issue. She wears belt for that and her bones are weak. Since the time she's discharged from hospital, she has this eyes problem. We showed it to two eye specialists too and they suggested an mri for brain and orbits. Mri reports attached for your reference. For to thyroid we had shown to Dr. girish in the past and he had prescribed neomercazole. She used to take 30 mg initially, 3 months months back her t3t4 tsh levels improved so he had reduced it to 10 mg a day. We increased it to 15 mg a day after since last 15 days after showing to a family doc. Our primary complaint now is eyes problem. Her vision has deteriorated and we put this refresh drops in eyes every 3 hours. Below are the thyroid reports that we have taken along with tsh receptor antibody. We have tried 3 injections of methylprednisolone in last 3 days. Still we are seeing no improvement. Tsh receptor antibody 38.68 done last month. T3 4.66 t4 1.46 and tsh 0.006medicine now tab tpo 100, predmet, azoran neomercazole 20 mg, apart from starpr press, metxl and clopitab a.what do you suggest?

Mr. lybrate-user I have gone through the history you have provided. It's quiet complicated. You have not mentioned age must be around 70 I guess. Is she having headache or pain in eyes. What is vision with n without glasses. Detail eye checkup is required. Specially rule out cataract, glaucoma. Hyperthyroidism has multiple eye involvement. All needs to be ruled out. In the mean time just use refres tears eye drops 3- 4 times a day. Specific treatment only after physical consultation.

I am 23 years male, I have hyperthyroidism before 6 months my weight was 54 kg. I have been taking carbimazole 40 mg tablet for 6 months and I took vitamin tablet for 1 month. With that for 4 months I took fruit salads (one apple, 2 small banana ,one pomegranate, some grapes, and quarter papaya) daily .it increases my weight rapidly. Almost 4 days a week I took it regularly .after 6 months now my weight is 74 kg .so nearly 20 kg was added throughout the process. And my thyroid level too decreased slightly coz I have been taking the medication regularly. My question is eating lot of fruits affects our health or not after 4 months I completely stopped eating fruits. And I have one problem. During sneezing I can't sneeze properly during sneeze my chest is paining ,this is due to fruits or what? And does that hyperthyroidism affect fertility or what. Now my weight is 74 .i am happy with that. During thyroid I lost almost 13 kg no I increased 20 kg.

Dear lybrate-user,
when you are treated ffor hyperthyroidism with carbimazole there will be weight gain as you have noticed if your tsh level is normal then your doctor will gradually reduce the dose of carbimazole. The sneezing problem you have may be due to an allergy problem in the nasal passage and if it is troublesome you can take an antihistamine like levocet 5 mg when needed
