I have question sir. Last week I did my hba1c test and fasting and after meal sugar test. My fasting was 81 and after heavy breakfast and sugar full tea my pp was 113, but my hba1c showed 6.8%. Next day only to confirm after heavy lunch in afternoon I did random sugar test in evening in lab and it showed 122. Then after three days again I did fasting and pp sugar test in lab and it was fasting 122 and pp 145. Then I purchased glucometer and every day since then im checking on glucometer, my results on it fasting 109, 120, 108,107, and pp 168,181,120,164, 150,145,182 like this. Am I diabetic? Can I take steps now to save myself. My family physician suggested to repeat hba1c after 3 months. Im taking statin for high cholesterol from last 1 year, is this because of statin? I have high bp only in doctors cabin but still my doctor prescribed telma 40 and nodon 2.5 but im taking only telma 40 from last 1 year. Please tell whats wrong with me.

Hello lybrate-user your sugar levels are not too much just modify your diet take diabetic diet and routine exercise and after that test for hb1c hopefully it will come to 5 to 6 .take care of yourself.

Due to chest and arm discomfort, high BP 155/110 & high heart rate 130 bpm advised ECG & Echo test. The findings are:- ECG: Sinus Tachycardia & Inferior MI (11, aVF) ECHO: Concentric LVH, LV Diastolic Dysfunction Grade 1 Doctor ruled out MI in ECG and the following medications prescribed: nebivolol 5 mg OD Cilnidipine 10 mg OD Cilnidipine 10+ Telmisartan 40 mg (night) Rosuvastatin 10 mg-od scheduled for Cardiology appointment after a month. Is the above condition that serious and medications need to be continued life long? The doctor has advised unless the BP is controlled aggressively there are chances of Ischemia or heart failure. Male: 47 years Height: 178 cms Weight: 91 kgs.

Please suggest me the clinical details of my echo as below: LA=5.5 cm, LV (diastole) =4.5 cm, LV (systole)=3.1 cm LV Function: LV end diastolic volume = 45 ml, LV end systolic volume = 15 ml, Stroke volume = 30 ml, Ejection Fraction = 66% LV posterior eall thk. = 0.9 cm, intraventricular septal thk. = 0.9 cm, aortic root = 2.8 cm, PA pressure 45mmhg, MVA by PHT = 1.7sq.cm, MG=6 mmhg. Please comment on any concerns? I am on nebivolol and digoxin. Stopped digoxin recently as per cardio's advice. Also I am on acitrom due to stroke affected a few years before. The question is Am I an heart failure patient? Please comment on any other concerns too.

As of now you have a good left ventricular function and hence your cardiologist has stopped digoxin but we should get a 2DECHO done after six months and plan accordingly.

I am a 40 years old male, weight: 51 kgs (ht. 5 ft 3 in). I am having high blood pressure since last 03 years. Presently under medication of cilnidipine 10 mg + nebivolol 2.5 mg once daily and my blood pressure is normal. But my serum uric acid often gets elevated for which I take febuxostat 40 mg. My cbc, lipid profile, liver panel, serum na, k values, fasting & pp bs, serum creatinine are all in normal range. Ecg impression is also quite normal, though 2d-echo shows mild left ventricular hypertrophy with diastolic dysfunction. My physician is advising me to switch to losartan 50 mg + cilnidipine 10 mg. What should be the best treatment for diastolic dysfunction/lvh? Should I switch to losartan-cilnidipine combination?

Yes you can switch to losartan and cilnidipine.
Losartan has benefits in reducing uric acid also. It will control bp and prevent remodeling of heart too. Adding cildnipine will further help to control bp by dilating blood vessels and reducing work load on heart.
The mainstay of treatment for lvh with diastolic dysfunction is ace/arbs, diuretics, ccbs and beta blockers!
beta blockers may later be added if needed!

I have been advised to take Human Mixtard insulin in the morning (20 units) and evening (15 units), besides other oral medications - in the morning: 1 Tab. Jalra M 50mg/500mg. For diabetes; 1 Tab Winbp 40H and in the evening: 1 Tab. Glycomet - GP 3/850, 1 Tab. Nebistar 5 mg, 1 Tab Ecosporin AV 75. Since I am on insulin, as it is, what is the rationale/need to have Jalra and Glycomet - since the metformin intake orally, exceeds 1300 mg per day?

Addition of metformin will restrict your insulin dose.You can stop glimipride in glycomet Gp 3/850 instead you can use glycomet 1000mg twice a day.You can stop jalra also because it is effective in early years of disease.

Other echo report is normal. My cardiologist says it is normal nothing to worry but keep BP under control. Presently my BP reads 130-140/80-85. My HbA1c is 6.20. Your doc advice your wisely. Just keep checking your sugar more frequently. Like once in three months or more. And have less salt. Like no pickles, chatani or any such things.

Nothing to worry. Just do regular exercise, either 30 mins exercise daily or 45 mins exercise for 3 days a week, and a healthy diet including vegetables, fruits except those sugary ones, low cholesterol ,low carbohydrates meal.

Sir, I am 69 years old male suffering from BPH since last 4 years and on Veltam-f tab for 3 years with no major reductions in symptoms. However painful gynecomastia developed on both breasts. Veltam-f tab stopped since last 3 months and started taking pumpkin seeds with persistent symptoms, but size of prostate increased from 30 cc to 62 cc. Please advise if rapid growth of prostate gland without increase in symptoms is a matter of concern asking for TURP? If not any tab in place of veltamf which will have no side effects.
