Hello dear, various cause may be
1)- may be she was infected before delivery that lead to meconium and premature delivery followed by retinopathy. 2)- due to some stress ful situations meconium was passed by baby and baby became prone to infection and after delivery retinopathy developed
premature baby are always prone for infections.

Yes it can be UTI again. Ideally antibiotics should be continued after the culture becomes negative. Some residual infection might have present and is becoming evident. Do urine culture and start her on antibiotics again asap.

"As" per your clinical history is concerned please follow like this -
1) Do a clinical examination by your Doctor[urologist] and to confirm do few investigations like this (Urine RE/ME/CULTURE & cystoscope) and take correct medication as per the report.
2) Do check your fasting blood sugar on tomorrow and if it is high then discuss with your Doctor to adjust medications and without medication adjustment infection will not be controlled.
