This appears to be autonomic dysfunction due to uncontrolled Diabetes.
Better control of sugars and better control of BP with vitamins and antioxidants to be given.
Adjustment of medicines dosage will also help.

Mr. K V Sethuraman, Your blood glucose levels are fairly good, within the recommended limits, particularly fasting glucose. What is the HbA1c%, it should be <7. If you have well controlled diabetes, then you can donate the kidney to a needy person.

Take your insulin in routine. Homolog before meals n lantus bedtime. What is more important you should check your sugar before n after exercise. If it is persistently high you can increase the dose of lantus by 4. 6 units. That won't increase the risk of hypoglycemia but if add homolog before exercise, chances of hypo will increase.
