It might be due to spasm of the abdomen muscles and pelvic floor muscles. It might also be due to gastric trouble as it generally happens during menstruation period. Hi if the patient has being on antibiotics for 3 to 5 days then there is no reason why the tooth cannot be extracted as we can extract a paining tooth but not an infected tooth that too in some cases .A good antibiotics coverage followed by extraction is the only option which I can see according to your description.

Kindly get an x-ray done of the tooth having pain. May be you will have to get root canal treatment done followed by a crown or cap. Till then you can take a pain killer like tab dentaforce dt 2 tablets dissolved in water. Brush your teeth twice daily using correct brushing technique to maintain your oral hygiene.
