Hello, Thanks for the query.
From the given information he is already on plenty of medications. A slight adjustment in Insulin dosage (increase by 2 units) and / or dietary adjustment would be more appropriate than adding any further drugs. What is fasting glucose as well as HbA1c%? At his age there is really no need for very strict glucose control. Thanks.

As you know it's poorly controlled and uncontrolled diabetes will harm her kidneys on the top of iga nephropathy. I recommend to start insulin for her with oral medicines. It's very highly unlikely to control her diabetes without insulin. Don't waste precious time.

No, you don't need to do anything else.
1. Just have calorie restricted balanced diet. Especially have salad in each meal and one seasonal fruit everyday.
2. Do aerobic exercise, ideally 150 mins per week.

Yes, you can change the medicines with suitable substitutes if the cost difference is huge. But, simply put the cost is often decided by various factors including quality.
