Good control is considered when FBG is <100 mg, PP is 150 to 160 mg & HbA1c%<6.8. (which is not mentioned). Your levels are still high. To bring down to desired values, you will have to give careful look at your diet and exercise. Bread (bakery items ) should be totally avoided. You may have to increase the intake of green leafy vegetables and salads. Plus take exact amount of calories based on your weight and physical activity. Thanks.

I want to know his HbA1c, duration of DM, height and weight to give good advice, any how his fasting is high he can change to T.Glycomet Trio 2 mg twice daily, Dynaglipt 20/500 morning after break fast, with regular walking and proper diet is essential. All the best.

Fasting glucose 224 mg is showing totally uncontrolled diabetes. It is showing that current drug therapy is inadequate. Needs to be upgraded. Plus there has to be a closer look at the dietary pattern and extent of daily physical activity. Hence, all those details are made available, it is not possible to give any suggestions. Dear, Yes your sugar levels are not well controlled and you are on a number of medications. But do not worry you still do not need to be insulin as we can still increase the doses of your medications. Glimepiride 1 mg can be increased so can met for min and pioglitazone. However januvia or sisal up tin is at the highest dose possible.

Januvia 100, metformin 500, bdid are for your dm. Odace n telma are for bp. Odace 5 mg tablet is used in the treatment of high blood pressure and heart failure. your attending physician must have given this because of your high bp. As both dm n ht are not good for heart.

Hello, Thanks for the query.
From the given information he is already on plenty of medications. A slight adjustment in Insulin dosage (increase by 2 units) and / or dietary adjustment would be more appropriate than adding any further drugs. What is fasting glucose as well as HbA1c%? At his age there is really no need for very strict glucose control. Thanks.

As you know it's poorly controlled and uncontrolled diabetes will harm her kidneys on the top of iga nephropathy. I recommend to start insulin for her with oral medicines. It's very highly unlikely to control her diabetes without insulin. Don't waste precious time.
