Please inform following details if you want appropriate advice: fasting, pp (after 2 hours), hba1c%, dietary pattern, extent of daily exercise, bp readings and lipid profile. I am 65 years and am controlling sugar since 25 years. Seeing slight increasing trend in HbA1C six months before (6.3 to 6.7), I consulted my endocrinologist and he increased dose of Glitaris M7. 5 tablet from half to one daily, and introduced one additional tablet Januvia 100 mg. This change brought down HbA1C back to 6.3. However, when I started taking Januvia, initially I had burning feeling in chest. It was temporary and I continued with Januvia. Recently (about one month back), I started having problem in swallowing solid food. I took a course of Gannamod tablets (20) and now I am practicing change in life style like keeping gap of 2 hours between dinner and bed. However, I am not quite comfortable with this and have developed anxiety for eating. I read that Januvia has side effect of acid reflux. Nevertheless, my endocrinologist told me that there is no such side effect due to Januvia and I should continue with the course. I request you to please confirm this.

Hello, Thanks for the query As you are on certain drugs your body gets adapted to those compounds. Therefore, some times the change in compound between similar drugs may nt give desired results. Trigenta is Linagliptin and Januvita is Sitagliptin, even though both have similar action, dosage is different. So there is a possibilty of different response. Results may improve over a period. The reason why blood glucose is not in control can be due to 1) Dietary intake is not as required or it is higher than recommended 2) No or limited exercise which does not burn body's glucose and does not help insulin action 3) Medicine taken may not be adequate. So one has to look at all of these causes and then decide. Thanks.

Hello, Thanks for the query.
The prescription is fine. What is important is to see that blood glucose should be brought down to FBG: <100 mg, PP: 150 to 160 mg & HbA1c% <6.8 (even <6%). That will help remain fit and healthy. Thanks.

Your fasting and PP glucose levels are two high. Which means current treatment is not adequate. There is a need to upgrade the same.
