Thank you Dr. insulin taken on 12-7-20: novomix 30/70--12 units before breakfast. 13 units novomix and before dinner13 units novomix. The results are: 13-7-20: fasting 136. Ppafter breakfast: 225…. Ppafter dinner: 239. Earlier my local Dr. put my on novomix 30/70 twice a day. Morning 22 and before dinner with 12 units. Butpps after breakfast and after lunch are around 225 to 250. After that local Dr. changed to 3 times novomix 30/70 with regimen 14,--14--13. Still pps after i and lunch are more than 225 only. My questions are :1) you stated novomix morning and before dinner with lantus. But this lantus is not taking care for 24 hours. My experience with lantus which is taken at night is working upto fasting level only. Due to my above experience, small doses of novomix after breakfast and dinner, will take care my high pp levels. For afternoon, you have not stated any insulin. Kindly review my case particularly keeping into account my high pp levels. Of course, if I take 3 times novorapid and onetime like tresiba, my levels may be alright. But 4 times punching my body with pain and often injections. 2) local Dr. stopped my gliptins like janumet or linagliptin when I stated losing of my muscle strength at thies area. As you stated, my injection area in abdomen or thies area not affected. If you say confidently, I can start any other medicine suitable to me if you suggest but my muscles should not be reduced. I am following strcy diet control with 1600 kilo cal. 3 months back my hba 1c is 7.4%. Pl advice and help me Dr. Regards prabhakara rao v vijayawada age 63 weight 73 kg.

Dear Mr. lybrate-user rao,
I note that the problem is with the pp values. Even on the present regimen, your hb a1c is reasonable particularly if you have had diabetes for > 10 years. I would suggest you try the follwoinf dietary changes:
if your fasting values are > 120 and pp values go above 180, reduce the quantity of carbohydrate (rice, wheat, rava, maida) in each meal and increase the quantity of protein (dhal, egg white, fish or chicken. Monitor blood sugars with glucometer as follo...more

Thanks for the query.
Both fasting and Pp blood glucose levels mentioned are very high. It appears that present medication (particularly Insulin) is not adequate. It may have to be increased. Plus a look has to be given to his diet and extent of daily physical activity. You may have to first target his fasting level and bring it down to 110 mg or so by adjusting nigh insulin dosage. Once the patient is on insulin they have to be on insulin, only dose may vary.Some times you may have to add medicines in addition to insulin.From the details you gave it appears you DM is very badly managed.So visit diabetologist once in a month and plan medicines and their modification this is a must.You should also manage your wt & diet which will help you to avoid complications & will assist you to get reasonable health.

During initial period of diagnosis, insulin helps to bring down the sugar rapidly and to reduce your problems fast. Later the insulin can be switched to oral tablets. Now the usage of tablets will be less if you can stick to your diet and exercise.
