You need to control your fasting and post lunch values with proper long acting insulin dosage -Insulatard and short acting insulin human actrapid insulin. You seem to be under weight for your height due to uncontrolled diabetes. Patients do benefit by taking Inj Insulatard twice in a day. You need to do regular physical exercise like walking yoga etc. Also control your diet strictly as per advice of your doctor if your blood sugar still remains high he may advise you some medicine or/and increase the dose of insulin.

15-20 mins before food. Depends on food intake. Hypoglycemia never comes alone with insulin, it's the food.
