You can take a basugine (glargine) substitute for lantus it comes in vial form and much cheaper.
Ask your diabetologist to shift you from rapid acting lispro (humalog) to regular insulin (actrapid) which is cheaper but you need to take it 30 mins before each meal as action is little slower.

If you are a type 1 patient you need to take insulin life long.
Humalog 50/ 50 (lispro) is 12 hr insulin so you need to take it 2 times daily.
50/50 means 50 % covers your breakfast sugars and remaining 50 % controls sugar for next 12 hrs.
If your breakfast and dinner is heavy then it suits you. You can do smbg charting with glucometer at home to check your sugar fluctuations. It will help to monitor your insulin dosage.

Take your insulin in routine. Homolog before meals n lantus bedtime. What is more important you should check your sugar before n after exercise. If it is persistently high you can increase the dose of lantus by 4. 6 units. That won't increase the risk of hypoglycemia but if add homolog before exercise, chances of hypo will increase.

every insulin has it's time of action. for example human mustard 30/70 has 12 hrs of action or actrapid has 6-8hrs , novorapid 4 hrs humalog 4 hrs humalog mix25/75 has 12hrs lantus (glargine) has 24 hrs etc..
after it is finished then sugars will start rising again and need another shot.

Normally starting dose of insulin for type 2 diabetes is roughly 0.4-0.6 units/kg weight. But in type 2 diabetes insulin resistance is variable. In long standing diabetes pt whose beta cells are exhausted and who is obese insulin requirement may go beyond 2-3 units/kg. HbA1c is approx average of blood sugar control in last 3 months. More HbA1c means more uncontrolled diabetes and may require more insulin.

Skin for most people is around 2.7 to 3 mm in thickness. So with 4 mm needles pinching might not be necessary since without pinching also the Insulin would enter subcutaneous tissue and not muscle tissue.
