The dosage of various medicines for glucose control can be adjusted on the basis of patients needs, doctor's advise and also based on dietary intake. Usually Glimepiride is either given once a day or twice in some cases. Metformin can be given 3 times a day again depending upon the need. Voglibose is usually recommended with each meal, that is three times a day. As it is mainly for reducing post food glucose rise. Thanks.

Hello, Thanks for the query.
It is difficult give a clear answer whether a SGLT - 2 Inhibitor can be added to existing therapy withput knowing full details of the case. I need to know the following: Fasting, PP glucose levels, HbA1c%, weight, height, serum creatinine & urea levels, eGFR and whethe there is microalbuminura. If kidney function is compromised or eGFR is low, then certain drugs can not be sude including metformin and SGLT-2 Inhibitors. Thanks.

When one takes adequate dosage of drugs, plus follows a strictly controlled diet and does regular exercise, blood glucose does get controlled. Therefore, it appears your present treatment is not proper. I need to know following things for giving any suggestions: fasting & PP glucose levels, HbA1c%, present dietary pattern and extent of daily exercise. Hello this practising geriatrician diabetes setting at younger age ie <35 years of age, tend to respond good to drugs at the start and in the later part, the insulin reserve will be low
would advise you to do c peptide, tsh and hba1c and creatinine if c peptide levels are low, would be better to add long acting insulin as an add on agent
need some more test to rule out other differentials for his symptoms like tiredness.

Fasting & pp glucose levels are very high, showing diabetes is not controlled at all. It means current treatment is not adequate at all, needs to be upgraded. Ideally fasting should be < 100 mg, pp 150 to 170 mg & hba1c% <6.8%. Therefore there is a need to give a re-look at over all treatment which includes drugs, diet and exercise. There is no mention of weight, height and diet so difficult to say anything further. Thanks.
