The details given are unclear. Since there is no mention of actual fasting, PP glucose levels,& HbA1c% there is no way to give any suggestions. Because by saying " sugar level & HbA1c5 are in good control" I do not understand what is considered as normal. thanks.

For a person with diabetes it is very important to keep the blood glucose in strict control, otherwise, the long term complications associated with diabetes affecting eyes (retina), kidneys, lower limbs, peripheral nerves and heart can occur more rapidly. Glycomet only has metformin. No glimepiride. Both the drugs good for diabetes. None of the drug cause kidney problem. In kidney problem we take some special care in prescribing metformin.

Since your glucose is quite well controlled and HbA1c% is 5.9 %, I do not see the reason for changing the treatment. In fact if diet is controlled by reducing a bit of carb intake, you will be able to achieve still better control. Even increasing physical activity would be more beneficial than adding any further drug. Tanks.

You are suffering from Type 2 Diabetes.
Medicine can be suggested only after we know the blood sugar levels, HbA1c & some other tests.

It appears you are on a fairly high dose of Glimepiride. Metformin (2000 mg) is also fairly high. Usually Glimepiride should not be used more than 4 mg in a day. Even though recommended upper limit is 8 mg, conventionally using more than 4 mg is not advised. No it will have one medicine less that is voglibose. Hence sugar level will increase. Discuss with your doctor who will add another medicine with different mechanism of action.
Control your diet strictly.
Exercise regularly.

Yes you can take insulin 10 units before dinner but which insulin it is humalog 30/70 but please check your sugar levels regularly for more information contact at my clinic or consult privately.

"As" per your clinical history is concerned please discuss with your family doctor about the nature of your disease [Type 1 and Type 2], if type 2 then you can take medication as mentioned and if type 1 then you can take insulin regularly.
