Hi
Your HbA1C is good but if there is any recurrent episodes of low blood sugars and tiredness, I would recommend you to reduce the medicine dose and mainatin a HbA1C of 6.5% - 7.0%. Your change of medicine and dose can be decided after a chat regarding your diet and exercise routine, daily monitoring of blood sugars as prescriptions cannot be given here.

Cost of knee replacement ranges from 1 lakh to 3 lakhs (average 2 lakhs) per knee, depending on the type of implant and hospital chosen. Rehabilitation can take 30 days to 3 months or more. Please send me a photo of your xray so that I can guide you better.

So, you're currently taking basal bolus with a dpp4-i. For some reason you're the doctor has not put you on metformin or other medications. Your weight is bound to go up since you're on insulin. I'd suggest you to get your blood workup done and visit a nearby diabetologist/endocrinologist to help you better.

Mr metformin can be taken up to 2500 mg per day. But 100 mg is also a high dose only. If it is not causing any gastro intestinal side effects then no problem. It is really good that you have started on insulin because looking at amount of drugs that you are taking, it is necessary for you to shift to insulin. By doing self monitoring of blood sugar daily you can increase the insulin dosage every alternate day till you reach target fasting level. It is good to reduce or take away pioglitazone.

You are suffering this because diabetes has taken a toll on your immunity. You need to strengthen your immunity and have better control over blood sugar. Ayurvedic tablets can do that without any side effects. Kindly let me know your age, height and weight so as to arrive at perfect prescription. Regards Dr. Omkar shahapurkar

Hello, thanks for the query.
I have seen the details mentioned. Your hba1c% is high, which means present treatment is not helping enough. Ideally in a well controlled diabetes patient's fbg should be < 100 mg or closer to 100 mg, pp 150 to 160 mg & hba1c% < 6.5%.
So there is a need to look at overall treatment that includes your dietary pattern, extent of daily exercise, present medication and need to upgrade, lipid profile, serum creatinine and urea (since age is not mentioned)

M lybrate-user, thanks for the additional query.
However, you seem to have not read the details that I had asked. With pp up to 190 mg, will normally not lead to such high hba1c% as it is the average level of fbg & pp over 24 hours. Plus there is no mention of dietary pattern, extent of exercise and lipid profile and age. It is essential to know all thedetails to gve any speidic advise. Howevr, if you have decided to take certain drugs on your own, you can go ahead. In this (open questions) ...more
