Hi muscle pain and fatigue could be due to your medications or it could be due to thyroid or vitamin d deficiency. Check your vitamin d levels, tsh, serum calcium, cpk, hb and esr.

But there is no information on blood glucose, (fasting & pp), hba1c%, lipid profile, dosage of medications being taken, dietary details and extent of daily exercise. With out having all the necessary information, there is no way to advise. what is your present diet?

Since your you have long standing diabetes and adding on acarbose my not achieve the target in my opinion basal insulin will help you more then acarbose.
