1) you can continue same dose of antituberculous treatment but if joint pain is severe you can take tab ibuprofen 200 mg sos (if you do not have any history of side effects to nsaids).
2) if still its not relived, taper dose of tab pyrazinamide.

Nop. Do not increase the dose before 14 days of treatment (4 days more). After that recheck blood uric acid level. If it is still high, then you can increase to 40 mg two times a day! thanks.
