Stop the roseday F. Roseday is needed AFTER heart attacks, and F should be prescribed in very rare cases, although it is very commonly used. You are getting into health problem of gout, so your present focus should not be on diseases you don't have. You should understand medicines that you have agreed to take long term without any disease. Don't make your health the responsibility of a Dr, it is your responsibility too.

Yes you can switch to losartan and cilnidipine.
Losartan has benefits in reducing uric acid also. It will control bp and prevent remodeling of heart too. Adding cildnipine will further help to control bp by dilating blood vessels and reducing work load on heart.
The mainstay of treatment for lvh with diastolic dysfunction is ace/arbs, diuretics, ccbs and beta blockers!
beta blockers may later be added if needed!

Hello doctor, I am 23 years old male and around 8 months ago, in my blood test I found high levels of creatinine and uric acid amd high blood pressure of around 170/90, then I went to kidney specialist and he prescribed febuxostat 40 mg for 3 months, and relmisart 40 mg twice a day 1-0-1, with amlodipine 5 mg once a day 1-0-0, now its been 6 months I am eating these, and stopped fexanto, and now due to lighten, from past 1 week, I am checking my blood pressure at home, so it is normal 115-80, hence now, can I reduce my dosage of telmisartan 40 mg to once a day, 1-0-0 and increase amlodipine dosage to twice a day, 1-0-1.because I have heard that telmisartan declines kidney function and is not good for kidneys for long term use. Ps: I cannot visit kidney specialist doc as he is stuck out of india because of cavity 19 lockdown. Please help me out with a detailed reply.

Hello, Thanks for the query.
I have seen the details given. I do not know from where you have gathered the information about Telmisartnan causing "kidney damage". In fact these group of drugs (ARBs) and ACE- Is are considered safest of blood pressure reducing drugs. Since your BP is well controlled, there is no reason to reduce the dosage. Telmisartan is shown to reduce proteinuria and help improve Glomerular Filtration Rate (GFR), which an indication of kidney function. Thanks.

I am 74 years young lady working as Adjunct prof after retirement. I hav been having Back pain for the last 40 yrs. Recently for six months I got knee pain also only on the left where I have back pain for the last many years. Recently in June I had strained for full two days of work of cleaning the entire house from top roof to down using ladder for going up and down. My knee got strained and swelling was there. I had consulted Ortho and took Xray and took tab for swelling pain and all for 20 days. In spite of that the swelling has not come down. I do few simple exercises taught by him/physio just once in a day for 15 mts. Swelling is still there and pain is also while walking but bearable .Now I am not sure whether to walk for 20 mts-long distances and move the leg frequently or whether it wd worsen the situation.. I prepare my food where certain movement or walk is to be done as fridge is outside the kitchen Dr. said that xray is ok and certain degeneration due to age. And asked me not to bother about swelling as it wd go after some time. Can it be due to uric acid which is high in my case as 9.1 .little chlorestrol Trigly, is higher I have started taking febustat Cd you suggest any herbal natural medicine. I stopped all tablets of pain killers and swelling killers. I put Mahanarayan oil on knee and give fomentation but not on regular basis. You need to be examined by surgeon and proctoscopy to be done by the time use ointment lox anally before defecation and ointment nitrogesic very little anally and qid and syp cremaffin 3 to 6 tsf hs keep stool soft take plenty of fluids orally.
