Cold compresses can help reduce swelling in the area. Cooling also helps to numb sharp pain. Apply an ice pack for up to 20 minutes, up to five times a day. Use a frozen gel pack, ice cubes in a plastic bag, or a bag of frozen peas. Wrap the cold pack in a soft towel. Do not apply a cold pack directly to skin.

Painful anus mean fissure in ano. Take gut clear 15 ml at bed time. Do sitz bath meaning to sit on Luke warm water for 15 minutes .3 times daily for 2 weeks. Apply anobliss oint around anus before and after defaecation and 3 times daily. Take cefixime 200 mg 2 times dailyv5 days. Take torcoxia bcd 90 mg once daily at bed time for 10 days.

Moreover treatment you received might not be appropriate for your symptoms or their might be some underlying chronic physical illness or psychosocial issues. You better consult other psychiatrist. The issue of long term medication is worry some. But there are long term studies in favour of medication.
