Hello, Thanks for the query.
Most of the drugs used in treating diabetes are fairly safe, on all vital organs including kidneys. Certain drugs are not recommended if the kidneys are already affected due to long standing diabetes and eGFR is low (<30 ml/minute). Thanks.

Mr Vasudev, Thanks for the query.
You are multiple drugs for controlling blood glucose. The reason these drugs have been prescribed must be improperly controlled blood glucose. Plus you are also on a drug combination to reduce LDL & TGL. There is nothing like high or low dose. Drugs are given based on the need. Since there is no mention of either fasting, PP glucose HbA1c% or lipid profile, it is difficult to know how good the control is. Thanks.

Yes you right in your comments on Jardiance, if the patient is suffering from recurrent UTI, withdraw the drug for time being and treat with topical as well as systemic antifungal, control of blood sugars by adjusting dosage of insulin, mostly in this pt low sugar may be due to insulin over dosage or poor intake and delayed intake, not because of Jardiance. After complete recovery from fungal infection restart Jardiance with minimal dose of 10 mg, to minimise the requirement of total dose of ins...more

Mr. lybrate-user, Thanks for the query.
With Jardiance there is a possibility of developing urinary tract infection on rare occasions. This tends to happen if strict precaution of washing the glans penis and prepuce (fore skin) is not taken. Because that drug excretes a lot of glucose through urine, so if proper washing is not done, then glucose remains on the glans penis, which helps bacterial and fungal growth. So it appears those precautions have not been taken. You may take necessary ant...more

Mr. lybrate-user Kumar, Thanks for the query.
While prescribing jardiance I am sure doctor must have given you instructions about washing the penis and prepuce cleanly so as to remove any glucose deposition that might occur there. The reason being Jardiance increases glucose excretion through urine. This helps in better blood glucose control and even weight reduction. But if care is not taken, then there is a strong possibility of developing fungal infection of prepuce and tip of penis area....more
