The condition occurs due to lack of lumbo pelvic rhythm that is imbalance in the muscle strength & length around lumbar and pelvic region.
Loss of spinal lordosis occurs due to posterior pelvic tilt i.e. tight abdominals and hamstrings + gluts.
Your problem can be cured ,Consult expert physio.
I would not suggest you any medicine as muscular spasm will come back when you will discontinue the medicine.

Currently, most likely you are having a fissure in ano and most of your other treatment is going well, except for hemorrhoids.
