As it can lead to a series of lonterm complications in relation with eye (retinopathy), kidney (nephropathy), peripheral neuropathy, problems with blood supply to lower limbs and coronary artery disease. You need to make special effort to bring sugar levels rapidly under control. You may need to shift to insulin injections right away, because looking at blood sugar levels it appears your beta cell reserve is almost nil. S...more

Hello,
Madam the glucose levels mentioned are very high, suggesting uncontrolled diabetes. This means 1) present medication is not adequate enough, there is a need to up grade the therapy. 2) one has to give a closer look at your diet and extent of exercise. 3) since there is no mention of weight and height, those details also need to be looked at. The doctor who is treating the patient is competent enough to adjust the dosage of the antidiabetic medicines...
Better let him handle the case...
For second opininon you can consult with full case details as in such cases we cannot suggest treatment on random basis.

Hello, thanks for the query. I have seen the details mentioned. The glucose levels & hba1c% are too high. Similarly tgls also too high. This suggests that current treatment needs an urgent upgrade ideally fbg should be <100 ng or close to it. Pp should be 150 to 170 mg & hba1c% <6.5%,tgl around 150 mg. Another important aspect is the weight you have grade 2 obesity with a bmi of >36.1 kg/sq meter. So there's a need to look at not only the drugs but diet, exercise & weight reduction. So please co...more

Hello, thanks for the query.
I have seen the details mentioned. Developing hypoglycemia in a patient with conventional insulin is fairly common seen problem. Plus if patient is also on glimiperide in addition to insulin, then there are greater chances. Since she was on 40 you + 28 u) two doses of insulin with 8 mg of glimiperide, the possibility was more.
Usually giving more than 4 mg of glimiperide a day does not really help. However, she was on 8 mg, which is too high. In addition ther...more
