Hello sir/madam. One month ago, my dad said that he is having pain in his shoulder, finger &knee joints then after few hrs, he was not able to move his hands & legs, then the very next day, we took him to the rheumatologist because our relatives said that these symptoms are of arthritis then the doc (rheumatologist & immunologist) told us to do lft test,& the result were like this- s.g.o.t - (65.0) s.g.p.t - (558.0) serum bilirubin total- (1.5) direct - (0.42) serum globulin - (4.5) then that doctor suggested these medicines - 1.hcq-300 2.urscol-30 3.tyzocort-12 after this he felt well & was able to move his hand & legs. Because of high level sgpt we took a second opinion of a doctor (gastroenterologist & hematologist) then he suggested these tests to us and so the results were- 1.lipid profile - normal 2.ceruloplasmin- 36.56 mg/dl 3.feritin- 387.63 ng/ml 4.ana- 0.3 5.anti lkm1 - negative <0.8 6.asma-positive (1+) 7.usg abdomen- normal then seeing these reports he said that it is probably - 1. Type-1 autoimmune hepatitis 2. Polyarthritis and told us to take these medicines 1.omnacortil-40 2.azoran-50 and told us to continue those medicines - hcq, ursocol, razo. But that 1st doctor said us that asma (1+) is may or may not be the aih type -1, and said to wait 10 days and told not to take omnacortil & azoran and said us to continue his prescribed medicines. After 10 we did our lft test as said the doctor, and the result was- s.g.o.t - (23.5 iu/l) s.g.p.t - (27.4 iu/l) total bilirubin - (1.4 mg/dl) direct bilirubin - (0.5 mg/dl) now my dad is well ,but I want to know that is he suffering from aih type-1? What is your suggestion after seeing all these reports? Only I want to know that is it aih type -1 or not? Hello sir/madam. Satrogyl is contraindicated in pregnancy.
Rest two Sulfasalazine, Azoran are reasonably safe meaning prescribing doctor comparing advantages and disadvantages may advise to take.
Folic acid must be taken during pregnancy in fact right from time when pregnancy is planned

Hi, patient has vision problems since last 1 month now. She has had problems in the past like breast cancer 7 year back. Only chemo was taken, no radiation. One breast removed. 2 years back she developed *hyperthyroidism* and she also had to put one stent. 2 months back she was hospitalized for back pain and mri was done and she had some *l4l5l6* issue. She wears belt for that and her bones are weak. Since the time she's discharged from hospital, she has this eyes problem. We showed it to two eye specialists too and they suggested an mri for brain and orbits. Mri reports attached for your reference. For to thyroid we had shown to Dr. girish in the past and he had prescribed neomercazole. She used to take 30 mg initially, 3 months months back her t3t4 tsh levels improved so he had reduced it to 10 mg a day. We increased it to 15 mg a day after since last 15 days after showing to a family doc. Our primary complaint now is eyes problem. Her vision has deteriorated and we put this refresh drops in eyes every 3 hours. Below are the thyroid reports that we have taken along with tsh receptor antibody. We have tried 3 injections of methylprednisolone in last 3 days. Still we are seeing no improvement. Tsh receptor antibody 38.68 done last month. T3 4.66 t4 1.46 and tsh 0.006medicine now tab tpo 100, predmet, azoran neomercazole 20 mg, apart from starpr press, metxl and clopitab a.what do you suggest?

Mr. lybrate-user I have gone through the history you have provided. It's quiet complicated. You have not mentioned age must be around 70 I guess. Is she having headache or pain in eyes. What is vision with n without glasses. Detail eye checkup is required. Specially rule out cataract, glaucoma. Hyperthyroidism has multiple eye involvement. All needs to be ruled out. In the mean time just use refres tears eye drops 3- 4 times a day. Specific treatment only after physical consultation.

Treatment for Churg?Strauss syndrome includes glucocorticoids (such as prednisolone) and other immunosuppressive drugs (such as azathioprine and cyclophosphamide). In many cases, the disease can be put into a type of chemical remission through drug therapy, but the disease is chronic and lifelong

You are on aspirin, which is a blood thinner. It causes excessive bleeding post extraction, so it should be stopped 3 days before extraction of the tooth and 3 days post extraction. You are also on immunosuppresant drugs i. E. Azathioprine and ciclosporin which is suppressing your immune system, to fight infection. You should be admitted, put on intravenous antibiotics pre and post extraction. You are also on wysolone, a corticosteroid. It should be doubled before extraction to avoid adrenal cri...more
