There are many reason for bad breath. If mouth wash is not effective then it may be due to nasal crust or nasal infection.
Try this-
tab cefixime 200 mg twice a day
tab cetrizine 10 mg once before sleep.
Nasal douching thrice a day.
×5 day
avoid if you have allergy with any of the above medicine.
Thanks.

Hello,
Halitosis, also called bad breath, is a symptom in which a noticeably unpleasant odor is present on the exhaled breath.
Of those who have genuine halitosis, often the odor is caused by bacteria present below the gumline and on the back of the tongue. Rest of them is accounted for by many different conditions, including disorders in the nasal cavity, sinuses, throat, lungs, esophagus, stomach or elsewhere.

Halotosis has many causes eg stomach ulcers, gingivitis. Chlorhexidine gluconate is a good option for post brushing in the morning and at night. Avoid using any other mouthwash during this period. If the problem still persists then I suggest visiting the medicine or dental department in a nearby hospital.
