Please get yourself examined by a surgeon. It may not be possible to avoid surgery for a fistula in ano. Get an MRI fisulogram done to visualize the entire fisula tract. You are likely to require Fistulectomy.

Hi Mr. lybrate-user, the medications you were prescribed are meant to control your symptoms. But the primary cause if any anorectal disease is mainly constipation followed by straining. So any thing you apply, problem may come back if you can't remove the causes. If the problem becomes too strong to interfere in your normal life style, then it is the time to ask about surgical options which will definitely give you long term and sustainable results. For best results complete fistula track has to be removed. You need to visit surgical gastoenterologist.

Pain and burning sensation after sphincterectomy shows your fissure has not healed completely. It is a commonly seen complication after surgery. Yes, it may be fissure in ano. You need proper medicine and hot sitz bath twice daily for 20 minutes.
