Hi, Had lung Tb with neck LN reactive lymph node hyperplasia at age of 9 years--completed full 6 months AKT. Now at age of 23-i have enlarged neck pevel 2 lymph nodes to size 3 cms-(painless, totally asymptomatic-no fever/nomalaise/no cough/no weight loss/tonight sweats) if I am diagnosed as TB lymph nodes-----will I be considered as category 2 AKT and will I need INJECTABLE streptomycin? Or I will just need oral drugs as it can be ATYPICAL mycobacteria as there is no symptom. Please help. Thank you.

Dear Lybrate user, you have to undergo FNAC of enlarged lymph nodes to confirm the diagnosis.
If TB is proved it would be better to add Injectable drug along with usual ATT.

Hello, I have pulmonary tuberculosis .i am 2nd time tuberculosis patient (i suffered from pulmonary tuberculosis in 2008. Now, My sputum test says 3+ afb detected and my Gene Xpert says - HIGH MTB DETECTED and NOT RESISTANCE TO rifampicin. I am on ATT (FIRST LINE WITH STREPTOMYCIN) from 1 month. BUT, I have very high grade fever almost 103 degree at evening. My blood test for typhoid, malaria, kalajar are negative. There is no weight gain in this 1 month after ATT started. My weight is 38, haemoglobin is 7 and RBC is 2.60,ESR - 98. This high grade fever irritates me and there is no gain in weight after 1 month of treatment. SO WHAT SHOULD I DO?

Kidly write detail of ATT with doses. Streptomycin have no role in retreatment . In most cases fever starts showing downtrends in 15 days, but in some may take long time. sputum should also be sent for afb culture and sensitivity to primary drugs.

I need help with this semen analysis report. I know it's not looking good. Viscosity - low pus cell - 2-4/hpf epith cell: ++ morphology: nf - 35%, af- 65% % motile: active - 15%, sluggish - 45%, dead - 40% - slow & progressive s/count - 10 million/ml culture - yielded moderate growth of staphylococcus aureus - strongly sensitive antimicrobial: ciprofloxacin, pefloxacin & streptomycin etc.

As semen shows less mobile proportions as well count is low too. Infection is there so start antibiotics for 10 days followed by repeat analysis and culture. Take expert opinion if needed.

I am 40 years old now. For 4 years now we have been looking for a child. I did this test recently and this is the result. Morphology 30%, abnormal >head defect 50%, tail 20%. Oh 8.0, motility: fast progressive 20% slow progressive 30%. Non progressive 20%, immotile 30%. Count: 13.2×10^6cells/ml. Normal range: 20-150x10^6 cells/ ml. Culture: yielded moderate growth of staph spp after 24 hrs of incubation at 37oc. Sensitivity: erythromycin +++, pefloxacin +++, ciprofloxacin++. Resistant: zinnacef, ampilox, gentamycin, septran, amoxicillin, streptomycin, rocephin. Please I need help.

1) accept high risk for mother and baby in view of age of mother if above 35
2) any couple desirous of pregnancy and not getting same naturally must meet gynecologist or infertility specialist accepting facts that it needs many reports and different trials of treatment. Means couple must have patience and go to one in whom they have trust to stick for a long time.
3) in view of age may select procedure giving fast result like ivf-

I am suffering from tuberculosis. After taking 6 month of medicine I developed tuberculosis lymphadenitis. Then I took on 5 drug of 1st line with streptomycin for 2 month since I had tb in 2008 in abdominal. I am taking medicine from 1 year but lymphadenitis still exist or even get bigger. My afb culture (specimen- lymph node) shows no growth, Gene x (specimen- lymph node) shows low MTB not resistant to rifampicin, my esr level is 40 mm/hr. I gained around 7 kgs between treatment 56 kgs to 63 kgs. But my weight is constant from 6 month not gaining. I am still suffering from low grade fever 98 to 98.6 in armpit. I am currently taking r cinex 600 and combutol 600 (2 tabs) per day. What should I do? My doctor said lymph node surgery?

You can stop all the medicines and get a lymph node biopsy. If positive restart akt and if negative then no need of treatment. Sometimes lymph node don't reduce in size beyond a certain size in tb lymphadenitis and they persist.

My mother has renal TB, 5 years back she had taken normal TB treatment for 6 months and there was an stent in ureters for 6 months. After 6 months the doctor said their is still swelling in ureters so you need an operation. So we decided not to do any operation. My mother stopped all medicine. But their was some pain in right kidney continuously. So we decided to show again to an urologist in june. The doctor said your tb is still not cured. The doctor said we need to put stent again in right ureters and start again tb medicine. So now to condition is there is an stent and my mother is taking tb medicine from 6 july. The urologist referred to govt tb hospital for tb treatment. My mother is taking cat -2 treatment with streptomycin. She had taken 60 injection. And now she is having vision problem, so tb expert said you need to stop ethambutol. So we stopped ethambutol. So I want to ask to you sir, that, should we stop ethambutol or not.

Yes on finding any evidence of Ethambutol eye toxicity, Ethambutol needs to be stopped immediately and not to be given again. An eye specialist should document the same. If required it should be replaced by another Antitubercular drug preferably with known sensitivity.

Hi, I have mtb from past 2.5 years I have lymph nodes first at chest operation done culture gene expert done I took r rcinex combutol sinclair and some other for 2.5 years after a 2 years I got one more swollen lymph node at my neck again the procedure followed by having tablets than culture gene expert which always shows not resistance mtb detected. In these 2.5 years I had two operations and non stop medications than I did ct chest 2nd time it gave good results Dr. stopped the medication now one more lymph node swollen at my neck again did culture and 3rd time ct chest shows I have mtb detected not resistance Dr. says I have to take streptomycin everyday for 2 months is it good and other question is I am working how can I get rid ofnthe pain that I will be getting after having daily injections and do I have to take other medicines with this injections or should I do biopsy the third time again please reply and also suggest a good Dr. for this its been high Time now please.

Hello doctor I was diagnosed by tb without cough and bloody sputum. I was started treating with forecox, omnacortil20,benadon 40 mg. After 15 days of treatment my sgpt increases to93. 6 and my doctor stops forecox and starts ethambutol 1000 streptomycin and I was treated three months. After 3 months I moved to another doctor and he started r cinex 400 ethambutol 1000 pyrazzamide 750. After some days I was got frustrated and take rcinex 400 10 tab at once and 13 tablets of trice 0.25 mg.after that I complaint of mild hepatitis. After 1 month I again started taking r cinex400 ethambutol 1000 pyrazzamide1000. This is my history. I want to know. Am I at risk of developing MDR tb. Please help me doctor .I am on ATT since march 30. My latest x ray and pleural fluid examination reports are attached. I left with att for 1 week and after that I eat ethambutol800 for 1 month. Actually means my tb medication disturbed for 1 month from mid June to mid July. I am very worried kindly help me doctor.

MDR TB is rare in extrapulmonary TB
TB medication should not be interrupted as in your case. Rise in liver enzymes is acceptable till 3-4 times rise.

Hello, I am suffering from tuberculosis from more than 1 year. After taking ATT for 6 months I developed tuberculosis lymphadenitis. Then I took on intensive tuberculosis treatment for 2 months with streptomycin in first line drugs. After this my tuberculosis lymphadenitis increase in size so I discontinue the treatment and go for better diagnosis. I go for AFB culture and Gene Xpert of lymphadenitis .My Gene Xpert shows "LOW MTB NOT RESISTANCE TO rifampicin" and my AFB shows" NO ACID FAST BACCI SEEN AFTER 6 WEEKS" After 1 month of discontinued treatment my medicine started again (R CINEX 600 AND COMBUTOL 600. I am taking ATT for lymphadenitis but my gland is still the same. I am 2nd time tb patient first in 2008. My chest xray is okay, ESR decreased to 20 from 40, I gained my weight from 56 to 63 kgs but I still have low grade fever 99-99.5.And what should I do for lymphadenitis? The size of largest lymph node is 37 mm×20 mm.

If size of lymph nodes have decreased or stationary with no TB toxaemia symtoms then continue TB treatment to complete minimum of 6 months treatment.

I had written to you on 11.03.2015. I have been detected for TB of the lower spine on 24th Jan 2015. Undergoing Anti Tuberculer Treatment since 28.01.2015. 24.02.2015 INH was stopped as I had failed Liver Function Test. On 04.03.2015 ATT was stoped as LFT failed miserably, Billirubin Total-1.32, Direct-0.70, Indirect-0.62 SGOT-868, SGPT-582 LFT on 09.03.2014 bilirubin total- 0.4, direct- 0.24, indirect - 0.16 SGOT (AST)-288.5, SGPT(ALT) - 669.3, GGPT-70.3, Still ATT discontinued. Following medicines were prescribed by doctor for one week. S-AdenosylMethionine 400mg 1 tab 2 times a day Ursodeoxycholic acid ip-450mg 1 tab 2 times a day Zevit- vitamin B complex and C with Zinc Sulphate - once a day Now the latest up-date is given below. 16.03.2015 the test reports are slightly better. Total ? 0.89 max 1.2 Direct-0.26 max 0.20 Indirect- 0.63 max 1.0 SGOT ? 91.3 MAX 35 SGPT ?- 225.5 MAX 46 following medicines were priscribed by doctor for one more week. S-AdenosylMethionine 400mg 1 tab 2 times a day Ursodeoxycholic acid ip-450mg 1 tab 2 times a day Zevit- vitamin B complex and C with Zinc Sulphate - once a day Also prescribed the following Ethambutol Hydrochloride-1200 mg-once a day Levofloxacin - 750mg once a day Refampicin - 450 mg - once a day Report dated 23.03.2015 This week again the test reports are much better. Bilirubin Total ? 0.96 max 1.2 Direct-0.42 max 0.20 Indirect- 0.54 max 1.0 SGOT ? 40.6 MAX 35 SGPT ?- 78.1 MAX 46 To continue with all the above mentioned medicines and he added Pyrazinamide 750mg 2 tablets in a day Further he also informed to take all the ATT medicines together after dinner. LFT dated 30.03.2015 Total ? 1.06 max 1.2 Direct- 0.33 max 0.20 Indirect- 0.73 max 1.0 SGOT ? 441.9 MAX 35 SGPT ?- 727.2 MAX 46 I do not know what the Doctor will say tomorrow as I have failed LFT again. Will ATT be stopped again partially or completely.
