Respected Dr, Sub:-Regd: Thyroid test values are TSH 19, T3 1.02, T4 5.43. I am 80 kg weight. I am using Thyro 50 mg. Is the dose sufficient or not pl advice me.

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Mr. lybrate-user, Thanks for the query. When a person is on treatment for hypothyroidism with adequate dose of levothyroxine, then TSH is maintained around 3 to 4 mU/L. That will make one free from all the symptoms of hypothyroidism. You have a very high TSH, which means that the disease is not in control. To reach the desired level of TSH you will have to take a higher dose of levothyroxine. Then test the TSH level after 6 to 8 weeks. If then level is in the above mentioned range, then the condition is considered as well controlled. Uncontrolled hypothyroidism leads to considerable weight gain. You have mentioned only your weight, but no height is given to find out your BMI. Please see that your TSH levels are maintained in the above mentioned range. Thanks.

Good to know that reports are normal and you are taking calcium supplements. Get an ultrasound abdomen. You will get more information about your gyne and liver and kidneys etc. With thyronorm, the basic metabolic rate increases, means your body is working more even when you are sitting, it's as if you are running and consuming more of your own resources. Take high protein, high calcium diet like toned milk and curds and boiled eggs plus proteins supplements plus seasonal vegetables and fruits plus eating according to constitution. Take more water as a routine. If joints hurt you but reports are normal, use less of ýour lower limbs. Remember our ancestors are monkeys who use all the 4 limbs but we humans use only 2 our legs, specially within kitchen also standing for hours without realising the load. No additional pain killers are suggested. Get massage to improve circulation of joints, any good oil will do plus hot fomentation with a heating pad.

Dear, Tobacco is addictive. Cigarette also contains 4000+ carcinogenic chemicals other than nicotine. The best method is to leave it and continue with your will power. There are other methods like Nicotine Replacement therapy and pharmacotherapy combined. If you?re ready to stop smoking and willing to get the support you need, you can recover from nicotine addiction and abuse abuse?no matter how bad the addiction or how powerless you feel. The first step in treating nicotine addiction is accepting that you have a problem. Confronting an addiction and accepting responsibility for your actions isn?t easy. But it?s a necessary step on the road to treatment and recovery. You should then consult a psychiatrist / psychologist who fill follow this treatment. 1. Detoxification using medicines. 2. Behavior modification with the help of therapies. 3. Counseling. 4. Medication to sustain and 5. Long term support. These given steps are essential for any addict to get effectively rid of smoking. Therefore, you need to stick to the treatment plan and cooperate with your psychiatrist / psychologist. Take care.

Focus on your diet: you really need to focus on your diet. When losing weight, many people feel that it is okay to eat what you want and then workout a lot. This is completely incorrect. The secret behind getting fit lies 80 percent in your diet, while the rest 20 percent lies in working out. This is an integral part in losing weight at home and being able to sustain it. Some of the famous weight loss foods. Green tea soya cinnamon multi grains wheat bran wheat grass lemon grass flaxseed pumpkin seeds sunflower seeds leeks lettuce broccoli olive oil quinoa aloe vera. Combine your diet with good physical activity. Fatty liver may cause enlargement of the liver, also result in symptoms such as fever, confusion and jaundice, or yellowing of the skin. Adopt a diet consisting of about 75 percent organic raw fruits, vegetables, seeds and nuts. Adopt a diet consisting of about 75 percent organic raw fruits, vegetables, seeds and nuts. Prescription for nutritional healing, raw, organic plant foods introduce fewer toxins into the body than cooked foods. This may reduce the workload on the liver, allowing it to clear out fatty deposits and regenerate healthy cells. Add turmeric to soups, curries. Turmeric, this spice contains a chemical, called curcumin, that may help protect the liver against damage caused by accumulated fats. It may also aid in the regeneration of liver cells and the removal of fats. The healing herbs, anise seeds may stimulate the regeneration of healthy liver cells, allowing the liver to flush out excess fats. Add beans and legumes, such as chickpeas, kidney beans, soybeans and peas, to your diet. These foods contain arginine, an amino acid that helps neutralize ammonia, which is a toxic byproduct created by the body during the digestion process. Detoxification of ammonia may reduce stress on the liver. Proper hydration may help cleanse the liver of fats and toxins.

I have an old stretch marks on my knee and on the back of my calf muscle for the past 4 years due to weight gain I have used variety of stretch mark oils and stretch mark lotions for almost six to eight months to remove them but there seems to be no difference. Is there any other solution to help me remove these stretch marks totally leaving no marks and not coming again with the cost effective remedy besides painful and expensive laser surgery.

Strectch marks occur due to sudden stretching or sagging of the skin. Stretch marks also depends on the elasticity of a person’s skin and the skin elasticity depends on the vitamins like vitamin E, omega 3 & 6. Strech marks take a long time to fade, so patience is the key to deal with stretch marks. To lighten the appearance of stretch marks – 1.Scrub the area lightly once a day. You can use some natural ingredients like gramflour, turmeric mixed with some milk to scrub the area. 2.Apply Vitamin E oil or warm olive oil on the area. 3.Apply Aloe Vera gel once a day on the area 4.Cocoa butter mixed with wheat germ oil is also very good at maintaining the skin health and reduce appearance of the skin. 5.You can also apply creams like Elastoderm stretch mark cream to reduce the appearance of stretch marks. Apply this cream daily to see results in few months. 6.Consume diet that is rich in vitamin E, omega 3 and 6 and other skin friendly vitamins like vitamin E and A. 7.Keep yourself hydrated. Drink atleast 8-10 glasses of water to keep your skin soft and supple.

I have blood sugar 130-170. Of late I have developed rashes in the body from scratching. In my thigh the spread is bigger. If it is due to sugar how many sugar patients get it. Remedy and reason please. Also my knee pains while climbing.

C.S.C, D.C.H, M.B.B.S
General Physician,
It is not as easy task to control blood sugar in the desired normal range. Whatever we eat is metabolised to sugar in the body to provide energy for our daily activities. For the absolute control, one has to monitor his / her blood sugar at regular intervals (self-monitoring of blood glucose? smbg). Though difficult, this can be done by a glucometer and keep a record. Get an idea of fasting sugar (fbs) and 2 hours after food (ppbs) and check before dinner and after every major food. The basic idea is to reduce the food intake to 1/3 of what you take now. The dictum of sugar control is diet, exercise, diet + medicine and at later stage medicine+ insulin will be needed. For sugar control there is the need for regular advice and follow up. Just one time consultation or advice is not enough to guide a diabetic in scientifically & medically correct manner. For giving you regular follow up you ask me privately with all details like your daily eating pattern and medicines you take.

You need to visit a psychiatrist immediately. This sounds like a serious condition, and you may be able to stave off worse developments if you act fast. Apart from the medication you must also seek help from a counselor for a good length of time. In the counseling you will learn a lot of skills and techniques to cope with the everyday challenges of life. If it turns out to be a pathological condition there is much work to be done and your cooperation is in your best interest. Do not self-regulate your medication, and comply with all the advice and prescriptions of the concerned doctors. When the mood swings to a better state, you may deceive yourself by thinking that you can now manage without help. That is a typical danger with this condition. Do not succumb to that belief and trust the professional. You could, if you want, seek a second opinion just for your curiosity.

Herpes - Am I At Risk?

MBBS .DGO.DA.(Trained in Andrology)
Sexologist, Ernakulam
Herpes - Am I At Risk?

Herpes is one of the most common STD of present times. Sometimes, there are people who are infected, but do not have any symptoms and can spread the infection to their partners. Genital herpes infection can be caused by two strains of virus, HSV1 or HSV2 (HSV for herpes simplex virus). It is spread by direct contact, from the site of infection to the site of contact. The infection usually presents as a sore (vaginal, anal or oral areas) and the fluid in these lesions contains the virus. When another person comes in contact with these sores (any region), then the infection spreads. This spread could be from any one contact area to another contact area.

When herpes is transmitted from people who are infected with the virus, but have no obvious symptoms, these are usually called prodromal symptoms, which include itching or tingling in the area of contact. The virus can spread from this area to the other person, and this form of transmission is known as “virus shedding.”

The infection can spread even during these initial stages until the sores are formed, to the time when they are completely healed and the skin looks normal again. Any sexual contact (vaginal, oral, or anal) can be risky and lead to the partner getting affected (75% chances of infection).

Asymptomatic transmission is also possible wherein the infection spreads when there are no symptoms in the infected person. In some people, the virus remains dormant for years together, but these people continue to shed the virus and are capable of infecting. This is more common with HSV2 strains than HSV1.

Pregnancy is another tricky situation where the infant can get infected through the mother. If there are traces of the virus in the vaginal tract, then it is safer to not have a vaginal delivery, else there are higher chances of the infant getting infected.

Newborns and/or babies can also get infected when someone who is infected kisses the baby. Given that their immunity is low, the chances of developing an infection are quite high. It has also been suggested that having cold sores puts a person at higher risk of developing HIV, though this remains to be proven.

Prevention:
Listed below are some ways to prevent or lower the chances of getting infected with the herpes virus.

  1. The most effective way to avoid STDs is to not have vaginal, anal or oral sex with a person who is likely to be infected.
  2. Get involved in long-term, monogamous relationship with a person who has been tested negative.
  3. With every sexual interaction, use latex condoms in the right manner. However, please note that condoms may not offer full protection from getting infected. If you wish to discuss about any specific problem, you can consult a Gynaecologist.
