Can a Sinus Infection (Rhinosinusitis) Cause Spinal Meningitis?

The answer is unfortunately. Yes!
Meningitis, sometimes referred as spinal meningitis, is an inflammation of the membranes surrounding the brain and spinal cord. Usually caused by a viral infection, but it can also be caused by a bacterial or fungal infection.
 
Among pediatric patients admitted for treatment of sinusitis, 3.2% were found to have an intracranial complication. Infection of the sphenoid sinuses, however, merits concern. These thin-walled sinuses develop late in childhood, and their deep location places them adjacent to the dura mater and other critical structures.
 
Sphenoid sinusitis is identified in approximately 3% of cases of acute sinusitis, typically in the context of pansinusitis. Significant development of the sphenoid sinuses does not begin until age 4 to 6 years, thus, sphenoid sinusitis is restricted.
 
Viral infection causes most cases of spinal meningitis. Viral meningitis is usually mild and heals without treatment. Bacterial meningitis is more severe and requires treatment with antibiotics. Streptococcus pneumoniae and neisseria meningitidis are strains of bacteria that cause pneumococcus and meningococcus meningitis respectively.
It can be life threatening condition owing to its proximity to brain and spinal cord and infective media is the fluid surrounding them.
 
Diagnostic tools:
• Lumbur puncture – csf examination (cell count, glucose, proteins) and culture
• Blood culture
• Chest x ray
• CT scan of head and nose – pns
• MRI brain
 
Symptom checker in meningitis secondary to sinusitis:
• Fever (92%)
• Headache (85%)
• Nausea, vomiting (62%)
• Altered consciousness (31%)
• Seizure (31%)
• Hemiparesis (23%)
• Visual disturbance (23%)
• Meningismus (23%)
 
Conclusion and quick pearls:
• Complications that are less common with antibiotics
• Orbital (cellulitis, abscess)
• Intracranial (subdural empyema, thrombosis of cavernous sinus)
• Bony osteomyelitis.
• Can result in drastic sequelae
• Drain abscess and open involved sinuses
• Ent surgical involvement – functional endoscopic sinus surgery
- Usually amenable with medical treatment
- Drain sinuses if no improvement after 48 hours
• Ophthalmology check up
• Neurosurgery intervention
 
A low index of suspicion is necessary for early diagnosis and treatment of sphenoid sinusitis, orbital complications and prevention of intracranial complications including meningitis.

Tuberculosis is curable or what? How long patient should take medications. Please mention some dietary foods for those patients.

Tuberculosis is completely curable. Duration of treatment depends on which part of the body it has affected and type of tb. A protein rich diet is must for tb patients. They should also take adequte amount of carbohydrate and fat. To avoid spices and refrigerated food items.

My grandmother is suffering from bone TB in spine l1&l2 from 6 months but from last 2 months she is not able to walk and sit properly for few seconds also, according to the recent X-RAY of spine conducted shows that her L5 of is collapsed and bone formation has started at l1& l2, what should be the remedy I.e. Medicine or operation.

Bone tb usually takes long time to resolve. But since she is suffering from activity limitation. She needs to have a team approach by a pulmonologist and ortho spine specialist. You can see me for the needful, she needs proper medical and if required spinal fixation.

Need throat swab examination both smear and culture and antibiotics as per result. Avoid cold drinks. Do saline gargles Antibiotics as per culture reports.

Hello doc. I recently completed my 18 month chest and abdominal tuberculosis treatment. Can I take beer now?

Occasional beer intake should not be a concern. But I would personally recommend you not to take any alcoholic beverages as one year post abdominal tb you just need to concentrate on building your nutrition by adequate protein intake.

1.You have to do steam Inhalation by a steam inhaler 2-3 times daily 2.Take Viscodyne LS syrup 2 Tsf thrice daily 3.You have to take antibiotic, use inhaler and do nebulisation along with it after consulting physician, if required4.consult physician for advice 5. If it is due to allergy do an allergy test after consulting Allergist/Immunologist to find out the cause of allergy and follow rhe advice and treatment accordingly.

I regularly have coughing problem specially in the morning. Mucus comes in my throat and causes continuous irritation till I am able to spot it out. Tried many medications. Often it triggers after something cold or during season change. Please advise.

Dear Lybrateuser, - Since you know the triggers try to avoid them - do warm salt water gargles twice a day if there is pain or irritation in the throat - have more of warm fluids like tea, coffee, milk, soup - avoid all cold fluids & refrigerated foods - take tablet levocetrizine, 10 mg, 1 tab daily in the evening - have a well balanced diet with more of fruits & vegetables, while grains & protein foods to boost your immunity - also avoid spicy, fried, sour & junk foods - have 7-8 glasses of water for proper hydration - do regular exercise like walking, jogging, swimming, also do yoga & deep breathing for 5-10 min daily morning to keep yourself fit & to boost your immunity.

I am a 21 year old gye, I have habit of smoking cigarettes. I smoke almost 6-5 cig. Sticks a day. Sometimes I suffer from cough. Especially in morning. But is is ok after some time. I am more curious about my sexual health till now I am not having any problem as such. But can this habit have effect on my sexual life in future?

Yes, smoking can do quite some damage to sperms, sperm production and to erectile disfunction. The vasoconstricting action of nicotine can reduce the flow of blood to the penis and lead to poor erection. So ideally you should give up smoking altogether. Smoking is never healthy for anyone.

I am facing some respiratory problem recently due to smoking. I am trying really hard to quit but I can't. What should I do?

Dear lybrate-user, Please try to stop smoking at the earliest. It has many health benefits in the long run apart from good free respiration. When you stop suddenly, you may get withdrawal symptoms like restlessness, irritability, lack of concentration, increased hunger, sleep disturbance, headache, impatience, anxiety, depression, etc. If you have them please don’t get back to smoking, take nicotine gum or lozenge or patch. They are available in all medical shops. You don’t need a prescription from doctors. Use 2 mg gum if you smoke less than 25 cigarettes/day. Use 4 mg gum if you smoke more than 25 cigarettes/day. First 6 weeks, use 1 gum every 1 – 2 hours. Reduce it in week 7 – 9 to one gum every 2 – 4 hours. In week 10 – 12, reduce it further to 1 gum every 4 -8 hours. By 3 months, you will be able to stop both cigarettes and gum fully without any withdrawal symptoms. Please feel confident that you can do it and give yourself some gift for doing it like vacation or books or new mobile. Smoking is a chronic relapsing condition like diabetes and B.P. So please don’t worry if you restart smoking. Start all over again to stop. You will learn the situations that make you smoke and avoid them. Please think of health benefits and money saved often. Plan how to avoid relapsing situations, like smoking friends, along with alcohol, stressful feeling or depression. Delay, drink, deep breathe, distract and discuss are the helpful tips. Increase of non smoking zones, gift yourself for stopping smoking. Even chewing cinnamon may delay. Please remember that the urge to smoke comes and goes like waves. Delay and see whether it goes away. It will like a passing cloud. You are young and you can try with these help.
