Hello Doctor, This question is for my 53-year-old mother-in-law who has been having rheumatoid arthritis for about 30 years. She has had bilateral hip and knee replacements, and her last operation was done about 5 years ago. Things were going fine until about 2 months ago when she started having severe neck and shoulder pain, sometimes radiating to her arms. We did consult her regular rheumatologist, who has asked her to begin Adalimumab injections, twice a month. We came to know that there are severe sideeffects to the drug and would like to know more about it and the success percentage of Adalimumab injection. It would be great if you could help us find other treatment alternatives that we can look up to.

Dear Radha, Namaskar, Adalimubab is a biologic drug,very effective in RA.......Ofcourse all the modern medicines do have some untoward and side effects.......but the treating Doctor has to titrate the dosage and choose the lesser evil out of Pendora of various drugs.... I am sure he will closely monitor the situation...There is no alternative treatment of this disease as for as scientific evidence is concerned.... Have faith and patience.... Take care.

Dear lybrate-user, get x-ray or mri done to rule out any structural joint abnormalities. Meanwhile, go for ayurveda's panchkarma therapy - we have many option for you - greeva basti (specialized procedure for neck pains), localised massgae with herbal oils, localised medicated steam, pottali massage. Continue these procedures for atleast 15 days. Regular oiling of the joint will improve the lubrication also. These procedures will relax your neck muscles and give strength to your nerves and muscles of neck. Also start some yogic exercises - tadasana, dhanurasan, halasana along with these therapies. Avoid using big pillow. Do not read or use gadets while lying on bed. This will greatly help.

Hello Lybrate user according to Ayurveda Vata vikriti is the main factor for pain. Treatment- 1-apply prasarini oil or pranacharya restopain oil on your affected part and give hot water fomentation. 2-take yograj guggul and agni tundi vati after lunch and dinner with warm water. 3-Take maha rasnadi kwath 2-2 tsf twice a day. 4-Take pranacharya vatari capsule and syrup twice a day. Diet- Avoid fermented food. Junk food. Potato rice. Sleep well.

Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Back Stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day. Do the cat/cow stretch. Get on all fours, with your arms straight and your hands directly under your shoulders; your knees hip-width apart.

Neck advice neck exercises. Neck stretching. Postural correction. Shoulder shrugs. Take frequent breaks at work. Core strenthening exercises. Use cervical pillow. Use back support. Self massage the back of neck gentle shoulder stretching. Core strengthening exercises. Gentle neck exercises. Avoid sleeping on affected side.

hello take high fiber diet. take luke warm water before and after meals. take green leafy veg, one fruit daily, plenty of water, avoid fried food , oily food, junk food, sugar, sweet, honey, rice,potatoes, white bread. include lemon water without sugar, coconut water with lemon , annar, salad, corn sup, broccali soup, veg juice, veg soup. do stretching exercise alternate days. do walk daily. don't skip your meals. have small and frequent meals.

Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Use no pillow under the head. Paracetamol 250mg od & sos any way take caldikind plus (mankind) 1tab odx10days x 5days do back (spine) exercises contact me again if need be. Make sure you are not allergic to any of the medicines you are going to take you may have to use lumbosacral support for some time. Avoid painful act/activities. Take medicines as prescribed it may have to be further investigated.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Actually decision in a knee depends upon a thorough clinical examination for all structures and then correlating findings with mri of affected knee. My take on this is: I think you should go for a diagnostic arthroscopy and then proceed. This will help in 2 ways, first we can see all structures inside ot with our own eyes and see for complete tears and laxity. Complete acl tear ar your age will require a reconstruction arthroscopically, lateral meniscus can be balanced and managed. Do not hesitate to contact me if you need any further assistance.

I am 50 years old, I have lower back pain ever since I was 10-12 years old, in the early stage I consulted doctors and they said that it will gradually reduce after marriage, but nothing like that happened and the pain was constently increasing. Today my pain is constant and I feel dizzy and start swetting if I stand at a place for more than 2-3 minutes. Normally I don't take any medicin, but if I feel that the pain is un bearable I take diclomax - 50 tablet, this happens once in abloom you can say 1 tablet per month, because I avoid taking pills I feel its better to bear the pain rathan than being adict to medicins. Can you suggest any other option rathar than medicin for relief of this pain, sometimes I use back ache belt while travelling anf while i am working in kitchen.

Kindly show me digital x rays of ls spine. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick matress are harmful use no pillow under the head. Do hot fomantation. Ibuprofen 200mg od & sos x 5days bio d3 max 1tab od x10 do neck, back & general exercises. It may have to be further investigated. You will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take. Do reply back for detailed treatment plan. Do not ignore. It could be beginning of a serious problem.

Hello, follow advises given below 1. Avoid using computers continuously, take rest for 5 min after working for half an hour 2. In pain free interval do neck exercise (kindly learn exercise by registered trainer) 3. You can use thin pillow while sleeping so that normal posture of neck can be maintained 4. Maintain proper posture of body while watching tv/ studying kindly go for cervical x ray ap and lateral and review with reports.

Hello lybrate user, back pain that is associated with day time sedentary jobs like computer professional s are associated with paraspinal muscle spasms. The best way to tackle this sort of pain is to do well spaced stretching exercises of the back, back extension exercises, using hot water bags to provide relief and avoiding stooping posture while sitting. Posture also plays a very important part in back pain, always maintain a straight posture while sitting. While getting up from the bed try to first go on your left or right side of the body and get up side ways. Avoid cruntches or sit-ups: they may increase the pressure on your back and may predispose to slip disc. Always maintain a healthy lifestyle in terms of regular brisk walking, meditation, eating healthy. P. S: try to avoid pain killers (analgesics) as much as possible as many are associated with side effects that you may not be aware of. Stay healthy. Stay happy.
