How To Treat Perthes Disease In Children?

The hip joint is a ball and socket joint made up of the round head of thigh bone (femoral head) with the cup shaped socket (acetabulum) of the pelvis and Perthe’s Disease is an affliction of the hip joints in growing children. It is much more common in boys than girls, and occurs most commonly in children aged between 4 to 10 years. The cause of this problem is still unidentified.

In Perthes disease, changes affect the femoral head which can be seen on X-ray. These changes occur in three stages over 18 months to 2 years:

  1. The blood supply to part of the femoral head is disturbed, causing loss of bone cells.
  2. Softening and collapse of the affected bone
  3. Re-establishment of the blood supply, repair and remodeling of the femoral head.

Limping is the most common symptom. The limp may become more persistent and pain may develop. Examination of the child by the orthopaedic surgeon generally shows restriction of hip movement. The nature of Perthes disease is variable. Severity depends on the child’s age, and the extent of femoral head involvement. Older children, girls, and those with greater involvement of the femoral head are likely to require more complex treatment. Treatment aims to reduce pain and stiffness, and prevent femoral head deformity.

All children need regular review by the orthopaedic surgeon through the duration of the disease. Not all children require active treatment. Many will make a good recovery with only symptomatic treatment. This may involve restriction of activity such as running and high impact sports. Swimming is encouraged. Some children may require exercise in slings and springs, or the application of plaster casts to the lower limbs. Some children will require surgical management.

Children with Perthes Disease are otherwise healthy, but may be affected by physical restrictions. By middle age, one third of those affected have no symptoms, one third have intermittent hip pain, and one third would develop arthritis requiring treatment.

Childhood obesity is a serious condition plaguing many adolescents and children these days. Obesity brings along with it a plethora of health scares such as high cholesterol, high blood pressure and diabetes, all of which at a point of time were confined only to adults. Obesity in children also results in depression and self-esteem issues. Too much of calorie consumption accompanied by a sedentary lifestyle and other hormonal or genetic factors contribute to obesity in children.

Overcoming Childhood Obesity-

  1. Healthy Eating: Consider opting for vegetables and fruits over high-calorie and fatty foods such as crackers, cookies or other processed foods. Cut out on the intake of sweetened beverages that makes your child too full to grab other healthy foods. Encourage sitting together for a family meal and initiating interaction rather than turning on the TV or other electronic gadgets. Allow your child to decide his/her proportion of food and never over feed. This habit should be inculcated in the child since the formative years.

  2. Engaging in Physical Activity: Make sure and talk your child into not spending more than two hours sitting glued to the TV set or the computer on a daily basis. Rather, egg him/her on to go out in the open and enjoy the day underneath the sun. Try and start building in your child the habits of exercising and staying active right from the beginning. This will go a long way in deciding your child’s health and overall fitness in the future. Remember! Morning shows the day. And it’s up to you to structure out your child’s ‘morning’.

Quick Tips For Parents-

Encourage:

  1. Play time or physical activities in ways that your child enjoys, such as going for a walk or hike, playing at the park, playing sports, dancing or going for a bike ride (and join them!)

  2. Healthy home-packed lunches

  3. Drinking more water and eating whole vegetables

  4. Eating when you are truly hungry, not when you’re bored, mad, stressed, etc.

  5. Eating together as a family

  6. Decreased meal sizes

  7. Focus on health rather than taste

Discourage:

  1. Too much screen time

  2. Eating while watching TV or other screens

  3. Eating out

  4. Negative food cues and triggers by removing them

  5. Skipping meals if you are hungry

  6. Eating if you are not hungry

  7. Eating by the clock (you should eat when you’re truly hungry)

  8. Too much sugary drinks (sodas and fruit drinks)

My son is 6 months old baby. E s not passed stool for two days and keep crying. I gave him gastica drops and dry grapes water also but still crying. I don know what to do please help me out. Can I give gripe water. Or tel me some home remedies or medicine to cure his stomach problem.

Hi Sir, my baby boy is 10 month old and weight 10 kg is having fever Dr. Suggested to take metal p tab 3/4 morning and evening. Is it advisable to take tablet 3/4 every 6-7 hrs if fever is recurring every 6-7 hrs. Please suggest.

If your doctor has prescibed it then Yes it can be given If the fever is high you can try putting a strip of fine cloth dipped in cold water or normal water on the forehead and body to reduce the fever.

My wife is a feeding mother to a year old toddler her breasts are uneven (left hand side) noticing from last 3-4 months, I'm afraid if there is any medical checkups to be done and from last 4 days she's complaining of sever pain in her left hand forearm but no swelling neither blood clothing is seen Do I have to be worried and also please let know whom to consult with!

Today I got my 5 and half months old baby vaccinated. Her thighs are paining and having very high fever. Habe calpol drops 0.5 ml twice but fever has not got. Please suggest me what to give now m having ibugesic plus n mankind forte too.

You can give about 0.8 ml every six hours (weight presumed 5.5-6 kg) & stay cool. Fever will subside within 48 hours. You can do ice fomentation over swollen part.

Upon reaching adolescence, children's bodies undergo several changes and so do their minds. It is then that their minds are most, as well as least impressionable. And the daunting question regarding the upbringing of an adolescent is how to treat their constantly changing behavior?

Here are some of the tantrums adolescents more commonly throw, and what should ideally be your approach to it:

  1. Your child seems to hate you: It is very common for a phase of emotional exclusionism to prevail between 16-17 years of age approximately. And the worst thing you could do is heighten that emotion by returning the hatred. Make sure you're firm against any extremely unacceptable behavior, but at the same time, show them that you're there for them no matter what.
  2. Electronic devices become the center of their attention: Whenever you want to have an earnest conversation with your children, their attention seems to be drawn solely to their phones and computers. They feel the need to be connected to their friends and all times. Ways to monitor that are by setting limit to the maximum hours spent on devices or making them pay their own bills, which will make them more responsible rather than splurging unnecessarily. Also, if your child is not entirely secluded from the family, it is probably best not to interfere all the time.
  3. Ignoring the curfew: Your kids often stay out later than the set limit. But it is quite possible that your curfew is unreasonable when compared to other parents' curfew. Find out what the average time limit is; it may prevent your child from bearing a grudge against you. In case they still fail to respect your curfew, make sure you spill out to them what the consequences can be, like being grounded for a week. However, in certain cases, your child may be spending time doing nothing constructive, but away from home. There may be something else going on, find out what that is.
  4. Being friends with the wrong people: Sometimes it might happen that you think some children do not have a good influence on your child, but you cannot say that directly because adolescents tend to get very defensive about their choice of friends. Unless the adolescent is doing something harmful with the friend, like using drugs, let him exercise his choice. Otherwise, don't hesitate from seeking professional help to counsel your child.
  5. Being over-dramatic: Every emotion is heightened in your child and you cannot tell them that what they are whining or being overjoyed about are trivial, because that's their prime focus then. Let them realize on their own how irrational they sound or else you may risk spoiling your relation with them.

There are a number of conditions that may come to affect the delicate skin of newborn and growing babies. Some of them may be simple to diagnose and treat, while others may be slightly more serious. Intertrigo is an exceptionally normal inflammatory condition influencing those areas of skin that are naturally in contact with each other. For example, the groin, armpits and other skin overlays. These territories of skin that are in contact with each other are in contact to the intertriginous zones.

Infants and babies with intertrigo have red and sore skin in the folds of skin, under the neck, under the arm and in the nappy area. Dissimilar to nappy rash, the rash is most dangerous, if it is found inside the folds of the skin. In a nappy rash, the skin inside a fold is usually protected, but the rash is exceedingly bad in areas touched by the nappy.

  1. In case you or your child has intertrigo, your doctor may propose just keeping the affected range dry and exposed to the air. You may control oozing of certain liquids with moist packs of an astringent called Burow's solution. Then air dry it and let it heal on its own.

  2. The surface of the affected skin is exceptionally sensitive and hard. It should be dealt with delicately. Washing it with water can be difficult. Utilizing sorbolene to clean the skin might be a gentler option. Be careful so as not to rub the skin. Pat it dry softly and regularly. Keeping the skin fold as dry as possible is very helpful.

  3. Creams or balms that shield the skin from dampness such as the ones that are utilized for nappy rash might be useful. Zinc creams, zinc and cod-liver oil creams can be helpful. Check with your doctor (drug specialist) for other valuable creams or salves. In case the rash is not improving or it is spreading, see your specialist as the contamination may require treatment. Thrush infections are normal. Utilizing a defensive cream like the ones used to counteract nappy rash are beneficial and effective.
