Dear Samshi, At an age of 42 the weight of 90+ is too high, the problem could be in the nerves or the vessels, and for both a thorough investigation is must, what you should know is that there are many times mass in the neck region which can cause compression on nerve and artery of hand coming from the neck and cause such symptoms. You should see a doctor urgently.

The scaphoid bone is one of the carpal bones in your hand around the area of your wrist towards the thumb side. It is the most common carpal bone to break (fracture). This bone broken usually by a fall on to an outstretched hand. Symptoms can include pain and swelling around the wrist.

Diagnosis of a scaphoid fracture can sometimes be difficult, as not all show up on x-rays. For a period of one to two weeks, and an mri/ct scan may be needed to ascertain the treatment.

Treatment is usually with a cast worn on your arm up to your elbow for 8 to 12 weeks. Sometimes surgery is advised. Correct diagnosis and prompt treatment of a scaphoid fracture can help to reduce complications.

Kienbocks Disease- A cause of slow wrist pain
Kienbock’s disease, or avascular necrosis of the lunate, is a condition in which the lunate bone, one of eight small carpal bones in the wrist, loses its blood supply, leading to death of the bone. The lunate is a central bone in the wrist, important for proper movement and support of the joint. The lunate, along with the adjacent bones on either side of it, the scaphoid and triquetrum, make up the proximal carpal row. This row of bones articulates with the 2 forearm bones (the radius and ulna), to form the portion of the wrist that provides the most motion. Damage to the lunate can lead to pain, stiffness, and in late stages, arthritis of the wrist. Kienbock’s disease is most common in men between the ages of 20 and 40 and rarely affects both wrists.

Treatment options depend upon the severity and stage of the disease. In very early stages, the treatment can be as simple as observation or immobilization. For more advanced stages, surgery is usually considered to try to reduce the forces on the lunate bone by lengthening, shortening, or fusing various bones in the forearm or wrist. Surgery can also be aimed at trying to restore blood supply to the lunate (revascularization), using a bone graft with a blood vessel attached to it. This is not an option in more advanced stages if the relationship of the bones has markedly deteriorated; complete wrist fusion may then be the preferred treatment. Hand therapy does not change the course of the disease; however, hand therapy can help to minimize the disability from the problem. Treatment is designed to relieve pain and restore function. Your hand surgeon will advise you of the best treatment options and explain the risks, benefits, and side-effects of various treatments for Kienbock’s disease

Low Back Pain- A common modern day epidemic
We regularly come across, frequent questions about back pain, and it remedy.
The concern of the patient is more related to the fear of something wrong, rather than the pain itself, which may be quite bearable. Before you go to consult to your doctor for your back problem, go though the checklist mentioned here, and you will be easily able to make out if your problem needs treatment or a good physiotherapy and life style modification.

Lets start with the statics.
1) back pain the most common reason for people to consult physician.
2) majority of them (and believe me about 80 to 85%) are functional which means they are not a disease but are due to improper lifestyle.
3) 10 to 15% of them are related to some pathology, and only these patients need medical help.

I know the next question in your mind, how do I know if my back pain need further consultation from a doctor, or I just want to need a physio and life style modification.

If you have any of these signs then you need consultation with a doctor.
1) duration of back pain more than 2 months.
2) localized back pain with stiffness in the morning,
3) pain not getting resolved despite good physio efforts and life style modifications
4) pain in the back with radiation to one of the legs, ie leg pain with back pain.
5) back pain associated with fever, appetite loss, or weight loss.
6) back pain with pain mainly in the night, and it arises you from sleep.
7) any association of weakness in the legs or numbness on walking.

Posture correction while work is the major remedy for postural back pain and this group is quite large.
1) use firm mattress for rest. Not a very soft one.
2) adequate support to the back while sitting on the chair, back should be kept straight but not strained.
3) regular back muscle strengthening exercises is must with this group.

When your back pain is not functional, then a consultation with your doctor is a must, most of the time the first doctor we consult is a physician, these days people are getting aware and consult an orthopaedic or a neuro surgeon. My advise is to consult a spine surgeon who are trained specially to deal with such problems. The usual examination is followed by certain blood tests and radiological investigations, like x rays and mri scanning. Out of these pathological type majority will not need surgery.

Keep strengthening your back, and prevent back pain. Remember physiotherapy (exercises) are to be done to prevent back pain and not to cure it.

Dear you could have injured a ligament in your knee, and this could be the reason for your problems, getting examined and investigated is the right approach to your problem, the treatment depends on your findings, and may be arthoscopic surgery to reconstruct your torn ligament will be the answer to your problems.

Dear your leg pain may be from your back, or due to deficiency, get yourself examined, get vit d and elemental calcium level done, and then get prescription on your condition. Empirical treatments are not advisable, and can even be dangerous.

Post Injury or fracture nerve palsy needs to be treated in time.
The usual period before which surgery should be undertaken so that good final outcome can be expected is around 3 months.

Dear, surgery in spine disorders is not a full proof guarantee that all will be well once surgery is done. As we all have multiple discs, so the key is to get surgery if pain is severe, and there are neurological weakness, but once you are off from surgery change lifestyle if you want no more problems.

Dear sir when I was born my one leg palm was tilted after consulting the doctor they plaster my leg for 6 months which result was good but my one leg becomes thin compared to another one and there was a difference of length 1-2 inch in the infected leg. Sir please tell me can I overcome with this difference as now I have attain my age 18.

You probably had ctev. The discrepancy stays but if you want length equalization that can be done but if you want increase in girth that' s not possible as muscle will increase with exercise only.

See life has no shortcuts and doctors and patients need to understand this alike. Pain reoccurring after disc surgery after 6 months indicate towards failure of life style modifications and physio on part of patient. Surgery gives no guarantee of permanent cure. You now need repeat MRI to see cause of your pain. The treatment depends accordingly.
