Your bp is high, sugar is in normal range and total cholesterol is high but get lipid profile done, get bp monitored and treated accordingly, reduce salt and sugar to bare minimum as fruits and vegetables give us enough of essential salts, minerals and vitamins. No added salt or packed food as salt is a preservative. Avoid excess salt as it leads to water retention and higher bp, but there is no need to take bland salt free diet, some additive to give taste is allowed. Take sendha namak as sodium is less in it compared to table salt.

Hi Mahendra, The basic difference between type 1 and type 2 diabetes is that in type 1 diabetes there is no insulin production be the beta cells of the pancreas and it is necessary to take insulin externally to utilize glucose, that is why it is also called Insulin dependent diabetes. While in type 2 diabetes there is defect in insulin functioning or/and insulin secretion, depending upon various conditions type 2 diabetes can be managed well with following proper diet, exercise and medication. Though in some cases patient with type 2 diabetes may also require insulin. Type 2 diabetes is majorly caused by poor lifestyle, excess body and is also hereditary while type 1 diabetes occur due to destruction of beta cells which is a auto immune defect. As per your description it seems that yours is type 2 diabetes.

Mr. lybrate-user, Thanks for the query. Hypothyroidism is a thyroxine hormone deficiency disease. This hormone is needed for every day functioning of the various metabolic activities in the body. Hence it has to be supplemented every day that to for lifetime. When on treatment with thyroxine TSH should be maintained between 3 to4 mU/L, then only one is free from all the symptoms of hypothyroidism. TSH 7.13 mU/L is high. It should come down to values mentioned above. So the dose of thyroxine has to increase. Thanks.

Many patients suffering from primary hypothyroidism ask whether taking long-term levothyroxine will have adverse side effects. Well, levothyroxine just replaces the deficiency of thyroid hormone in your body, it does the functions which thyroid hormone would have done in your body if you did not have a thyroid hormone deficiency. So the question of side effects doesn't arise, as its just a replacement of what your body is deficient in. However, a dose of levothyroxine needs to be titrated before maintaining TSH within normal limits. After initiation of thyroxine, TSH is rechecked in 4-6 weeks and dose adjusted. once TSH COMES IN NORMAL RANGE , thereafter it is checked on an annual basis and dose altered if needed. PLEASE DO NOT STOP , INCREASE OR DECREASE LEVOTHYROXIN without asking your doctor.

I am 72 years old male with diabetes since 25 years. My diabetes is under control by medicines and insulin injections. My question was how to improve my haemoglobin level which is around 9 since a few years. The answer I got has nothing to do with haemoglobin but only regarding diabetes, which is well under control.

Infertility - A Complete Guide!

The term infertility in females explains a wide range of disorders but in order to simplify things, lets’ just say, infertility in women is a condition in which women are unable to conceive. There are numerous reasons that can be responsible for this situation and although some are preventable or curable, most others have no specific cure or treatment.

Ovulatory Disorders

One of the most common reasons for infertility is an ovulatory disorder. Almost 30 percent of female infertility is caused due to this reason. 70 percent of such infertility is treatable with drugs such as Reprones/Menogan and Clomiphene. Here is a list of causes of failed ovulation-

  1. Disruption in the complex hormonal balance.

  2. Ovaries fail to produce mature eggs. Polycystic ovarian syndrome is the most common disorder causing this problem. This syndrome has reduced FSH production and increased production of LH, Oestrogen and Testosterone. Suppressed FSH production may cause the partial development of ovarian follicles.

  3. Hypothalamus may malfunction thereby causing the pituitary to malfunction, thus losing control over the process or FSH and LH production.

  4. Physical damage to the ovaries by multiple surgeries or due to the formation of cysts.

  5. A rare case of premature menopause.

  6. Problems of the follicle.

  7. Poor functioning of fallopian tubes

Diseases or disorders in the tube have been another major reason of infertility in women. Almost 25 percent of women infertility occurs due to this reason. Although treatments exist, success rates of the treatments are as high as 30 percent. The causes of tube damage may be as follows-

  1. Infection caused by bacteria or viruses

  2. Abdominal diseases like colitis and appendicitis

  3. Previous pelvic or abdominal surgeries.

  4. A condition called ectopic pregnancy which occurs in the tube, and even if very carefully but successfully overcome, may leave permanent damage.

  5. Rare congenital defects in which women may be born with tube anomalies.

Endometriosis

This is the condition where the endometrium expands excessively thereby preventing individuals from getting pregnant. This affects almost 10 percent of the population of infertile women. Almost 40 percent of women with endometriosis are infertile.

Other Factors

Other additional factors may include:

  1. Abnormal uterus

  2. Congenital diseases

  3. Behavioural factors like personal habits, lifestyle and health factors.

  4. Exercise and diet- extremely overweight or underweight women may have problems in conceiving.

  5. Cigarette smoking reduces the chances of conceiving by one-third.

  6. Alcohol increases the chances of birth defects.

Hence, pregnancy is tougher than it might really seem. If you wish to discuss about any specific problem, you can consult a gynaecologist.

Your fasting levels are within normal range. Maintain your weight, BMI below 23. Take regular exercise and take healthy diet. Limited junk food, fried foods, refined carbohydrates. Take plenty of salad. Stress is another factor. First of all relax you will not get if you keep life positive.

I drink more cool drinks I have thyroid problem already. Is intake of more soft drinks cause any problem?

Soft drinks may not affect thyroid directly but it predisposes to many other problems like gastritis, diabetes nutritional imbalances which aggrevate any exhisting disease. Please stop have softdrinks as they are as bad for health as alcohol drinks.

I am male-61 yrs. I hv diabetes and also facing problems of water blisters (diabetic bullae) on my legs/hands recurrently. My wt is 79.8 kg, My HBA1C is 7.5. You have not mentioned your height. For diabetes you need to avoid sugar, jaggery and honey. In fruits you need to restrict banana, chickoo, mango, sitaphal, watermelon, sugarcane, muskmelon, grapes etc. Yo0u need to avoid deep fried foods like farsan and namkeen etc. In vegetables you need to restrict potato, sweet potato, yam, beetroot and carrot etc. Try to consume more of salad, Vegetables and whole grains. Eat small and frequent meals. Do not consume any of your meal heavy. Start taking fenugreek seeds whole or powder in the morning as it will help you keeping your sugar up to normal. Do more of yoga, breathing exercises and weight loss exercises under expert's observation. Your HBA1C should be 5 or below.

These are for diabetes and vitamin tablets ( pregalabin ). If you have diabetes at this age you are type 1 diabetic and insulin is a must. The side effects are different for each tablets but not serious to stop tablets as controlling diabetes is the most important aim to avoid umpteen complication of it. Need not worry about side effects ans a all people have these without any bad effects . you can consult me for private guidelines

I test whole body check up 2 month before where my ggt is 195 sgpt 44 sgot 29 alkline phosphate 142 and all test are normal yesterday again I test it now ggt 165 sgpt 37 sgot 26 alk phospaye 117 but this time total cholesterol 234 red cell (rdw-cv) 16 red cell (rdw-sd) 55 I do not know what can I do I leave alcohol from last 6 month I have no problem except sometime very little pain at right side near liver can you suggest what can I do for normal ggt and what is red cell?

Dear, Your ppg is very less and your creatinine and urea are high. Your creatinine clearance is only 48.49, means very less. So I will like to advice you treat very aggressively, as your kidney is affected more. It should be more then 60. So please consult to good diabetologist and nephrologist for dm and kidney. As your kidneys are affected, you have to reduce the dose. Please inform me your all medicines you are taking. You may be having type 1 dm.

For a person with hypothyroidism foods containing soya and soya containing products, cabbage, cauliflower, broccoli, mustard should be avoided, along with coffee. Should consume more green vegetables, salads, fruits and use whole grain flour for making chapatis and rotis. In addition regular exercise is equally important.

Take diet for hypothyroidism:- high-carbohydrate foods such as potatoes, rice, pasta and bread, Nutrient-rich foods that improve your health may also benefit your thyroid gland, antioxidant-rich fruits and vegetables more plenty of water: Blueberries, tomatoes, bell peppers, and other foods rich in antioxidants can improve overall health and benefit the thyroid gland. Eating foods high in B vitamins, like whole grains, may also help. flax oil, Selenium: Tiny amounts of selenium are needed for enzymes that make thyroid hormones to work properly. Eating selenium-rich foods, such as sunflower seeds or Brazil nuts, can be beneficial. Tyrosine: This amino acid is used by the thyroid gland to produce T3 and T4. Taking a supplement may help, Avoid few foods in hypothroidism i.e. caffeine products tea,coffee, tabcco, alcohol / wine , kale, broccoli, spinach, mustard green, cabbage, turnip, millet, cassava, linseeds, pine nuts, soy products as soy milk, tofu, soybeans, soy souce, also avoid stress. Take Some Relax during work. Proper follow up you can feel better and reduce weight also.

I was border line diabetic. Open heart surgery done 11 years back. Was on anti cholesterol med. Suddenly, it's about a year now, I became anti diabetic, meaning acute hypo glycemic, meaning need to eat sugar time and again, as sugar lowers to 60 at times, resulting in over weight. Suggest.

My dad is around 45 years old and yesterday he went for a blood sugar checkup it came 356 before food and some 600 afterwards though he had no fever or nothing just some tiredness and leg pain. I know that the level is very high and he started medicine immediately. Now I want to ask is this high blood sugar level same as diabetes and is it life threatening? What should be the immediate steps that we should take? please reply.

It is not life threatening. He has to continue medicine. Follow the advice given below 1. Do regular aerobic exercise for 1 hour daily (brisk walking, jogging, running, swimming, cycling etc) 2. Reduce weight if overweight 3control diet a) avoid sweets, sugar/honey, milk, milk product excess calorie and carbohydrate rich diet like white bread, cereal, rice, pasta b) avoid all refined food like maida, pasta, starchy food like rice, potato, bakery item, processed food, c) include more fiber like pulse, green leafy vegetable, cucumber, carrot, tomato, j legume. D) avoid sweetened fruit like banana, mango, litchi, chickoo, sapota, seethaphal jack fruit, grape etc. Can have fruits like guava, pineapple, papaya, orange, mousambi, strawberry, watermelon, pomegranate, jack fruit, sweet potato, a handful of walnut daily (akhrot) e) avoid white/polished rice, take unpolished rice like brown rice/wheat/jowar/ragi, moong and chana dal. F) avoid ghee/ butter./coconut oil/excess salt intake like papad/pickle. G) avoid red meat. Take fish/chicken. H) avoid alcohol and carbonated drinks like soft drink 4. Adhere to a strict diabetic diet by consulting dietitian 5. Monitor your fasting, pp blood sugar 3 monthly and hba1c six monthly 6. Consult diabetologist for advice and treatment.

Mr. lybrate-user, If the patient is regularly taking prescribed medicine and also following diet restriction and exercise schedule, then blood glucose does come down. That is what is seen with the patient. It means due to current medication diabetes is under control. But to remain in that good control, treatment has to be continued for lifetime. Plus periodic check ups should also be done. If you stop medication now, blood glucose levels will go high and can cause severe problems. Thanks.

I am 23 years old guy with weight 59kgs and height 5.9' feet, Recently I have taken Thyroid test where my results are T3-124, T4-10.9, TSH-6.32. I was skinny, less hemoglobin in blood, Whatever food that I take is getting waste (No proper digestion), heavy hair loss, weak body. These are the problems that am facing Kindly suggest me necessary prescription.

Your thyroid test results show that tsh level is slightly higher than normal limit. It is classified as sub clinical hypothyroidism. Usually treatment is initiated when tsh level crosses 10 milli units / ml. Your bmi is 19.28 kg/m2, which is low. You need to take good balanced diet, exercise regularly, increase the caloric intake as well as protein quantity to build muscle mass. You should avoid soy beans, soy containing products, cabbage, cauliflower, broccoli, radish, peanuts, mustard and coffee. Consume more of green leafy vegetables and salads and fruits. Always use whole grain flour for chapati and roti, do not use maida.

Hi Doctor, I am 60 years old working person. I am diabetic since 2003. Taking Diabetic medicine - Amaryl - 3 mg, Galvus Met 50/1000 mg, Neurobion Fort including Herbal Medicines - like - Karela - Jamun Juice 30 ml, Aloevera Gel 30 ml and 2 coves of Garlic - empty stomach in the morning. Also no Sugar intakes. Using Zero calorie sweetener. Even then my FBS results are coming High between 150 - 170. Can You please advise something from your side.

This is dawn phenomenon. Or it could be somogi effect. Get blood sugar tests between 2 to 4 am if low it's somogi otherwise it's dawn phenomenon take 2 biscuits marie at midnight and see results after 2 weeks. Do brisk walk early morning on getting up before doing blood sugar fasting test. Consult again.
