I am 38 year old. I am non-smoker, non-drinker and vegetarian. I have led a very healthy life. However, all of a sudden fell weakness and pain in my hands and leg; got my blood sugar checked and got to know I have diabetes. Fasting was 208 and pp was 370. Since then, consulted doctor, am taking medicines regularly, walk of 1 hr a day, I have discarded sugar from my life. Still, I am unable to bring my blood sugar to normal levels. Is my condition permanently curable? if not, can you suggest few remedies, diet, lifestyle; medicines or products; to lower the diabetes risk to minimum. There are several herbal/ayurvedic products to combat diabetes available thru tv shopping networks. Are they beneficial? thanks.

Sorry to learn that you became a diabetic in spite of your healthy lifestyle. In most people, diabetes occurs as a combination of hereditary factors, life style, diet & over weight. There's nothing you can do about heredity, right? Thus, in spite of a healthy life style, your genes got you diabetes! Unfortunately, diabetes is NOT permanently curable. If you diabetologist's medicines, your lifestyle & diet have got your sugars well controlled, please continue. In the meanwhile, get evaluated for problems involving your eyes, heart, kidneys & feet. Please do NOT go for so-called herbal medicines sold thru TV shows.

My thyroid test was done today and it was 10.44 range and I was taking 50 mg medicine earlier from tomorrow do I need to increase the power of tablet to 100 mg. Kindly suggest.

MBBS, CCEBDM, Diploma in Diabetology

lybrate-user, Thanks for the query. TSH 10.44 mU/L, shows hypothyroidism is not in control. TSH should be maintained around 3 to 4 mU/L. So you need to increase the dosage. But dose should not be increased by such a big margin. Increase has to be gradual. So my suggestion is increase by 12.5 mcg per day (50 + 12.5 mcg=62.5 mcg). Then check TSh after 6 weeks, if the level has not come down to the expected value, you may have to increase the dose by another 12.5 mcg and again TSH has to be checked after 6 to 8 weeks. Once you achieve 3 to 4 mU/L, continue the same dose. Thanks.

Do not worry first get your blood sugar levels under control and rest will be fine.

Hi, Minimum 10-12 hours of fasting is necessary to check FBS. Therefore have your dinner within 8.00 PM. Check you PPBS 2 hours after the last bite of your lunch or breakfast. Have regular breakfast or lunch; but eat stomach full and then go for the blood test.

pp level of 180 mg is high, fall in pre diabetic stage. You have to continue what you are doing now, plus exercise regularly. Hba1c% of 6.6% shows diabetes level. To be on safer side it is better to consider taking a drug like metformin along with diet, exercise and weight control.

If your brother's blood sugar levels are well controlled, there's no harm if he takes one medium sized mango daily. By the way, who told you that mango" reduces" insulin level?

What causes Hashimoto's thyroiditis, and what is the best method of treatment? Can iodine help this condition?

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Mr. Lybrate-user, hashimoto's thyroiditis (ht) is an autoimmune disease, with lymphatic infiltration of thyroid gland. It becomes firm or globular. There are anti bodies produced against thyroid peroxidase enzyme (anti tpo ab) which destroy thyroxine secreting cells resulting in hypothyroidism. Treatment is to give thyroxine tablets. Iodine has no role in the treatment. In fact one of the causes is supposed to be taking extra iodine through iodinated salt.

What are the symptoms of diabetes. And what kind of solution for that we should take.

C.S.C, D.C.H, M.B.B.S
General Physician,
What are the symptoms of diabetes. And what kind of solution for that we should take.
Diabetes affects all organs in the body – Eyes, kidneys, nerves, reproductive system all are affected, In fact, from toe to hair. For diabetic control, patient and doctor should be in constant touch for regular follow up and advice. The common symptoms are :-Both types of diabetes have some of the same tell-tale warning signs. Hunger and fatigue. Your body converts the food you eat into glucose that your cells use for energy. But your cells need insulin to bring the glucose in. If your body doesn't make enough or any insulin, or if your cells resist the insulin your body makes, the glucose can't get into them and you have no energy. This can make you more hungry and tired than usual. Urinating more often and being thirstier. The average person usually has to pee between four and seven times in 24 hours, but people with diabetes may go a lot more. Dry mouth and itchy skin. Because your body is using fluids to make pee, there's less moisture for other things. You could get dehydrated, and your mouth may feel dry. Dry skin can make you itchy. Blurred vision. Changing fluid levels in your body could make the lenses in your eyes swell up. They change shape and lose their ability to focus. Diabetes, often referred to by doctors as diabetes mellitus, describes a group of metabolic diseases in which the person has high blood glucose (blood sugar), either because insulin production is inadequate, or because the body's cells do not respond properly to insulin, or both. It is a difficult task to control blood sugar in the desired normal range. Whatever we eat is metabolised to sugar in the body to provide energy for our daily activities. For the absolute control, one has to monitor his / her blood sugar at regular intervals (Self-Monitoring of Blood glucose – SMBG). Though difficult, this can be done by a glucometer and keep a record. Get an idea of Fasting sugar (FBS) and 2 hours after food (PPBS), & HbA1C (inform me) and check before dinner and after every major food. The basic idea is to reduce the food intake to 1/3 of what you take now. The dictum of sugar control is diet, exercise, diet + medicine and at later stage medicine+ Insulin will be needed. There are different diet plans of varying calories for each person depending on his activity & life style. For diabetes, hypertension (high BP) and many long-term illness control there is the need for regular advice and follow up. Just one time consultation or advice is not enough to guide a diabetic in scientifically & medically correct manner. For giving you regular follow up you ask me with all details like your daily eating pattern and medicines you take. I assure you a proper guidance... if you keep in touch personally.

Please clarify whether Honey and jaggery / jaggery items can be eaten by the diabetic patients.

Please clarify whether Honey and jaggery / jaggery items can be eaten by the diabetic patients.
Honey and jaggery should ideally be avoided by diabetic patients. However, as in some vegetable or dal preparations, if it has to be used, it is better to use these instead of sugar. Sweets made of jaggery or honey should be avoided.

Mr. Lybrate-user, t2 diabetes when goes on over 10 -15 years does tend to cause a number of long term complications. These tend to occur early if blood glucose control is not good. Persistent high blood glucose damages peripheral nerve endings leading to altered sensation and a patient feels tingling, numbness and burning sensation in the lower limbs, more so in feet. One of the primary requirement is to maintain a strict blood glucose control with fbs closer to 100 mg, pp 160 to 180 mg & hba1c% <7. There are various medicines used to improve this condition like vitamin b12, lipoleic acid, folic acid combinations and drugs like gabapentine and pragabalin. Thanks.

Hello doctor one day l'm checked own sugar in monitoring system and show result'low'9-14. It means that hypoglycemia....
It is better if you get your blood sugar checked in a close by laboratory, that will give a clear idea. If diabetes is present then readings are: fasting >126 mg and pp >200 mg /100ml. If fasting sugar is between 100 to 125 mg it is known as fasting glucose intolerance. If pp is between 140 to 199 mg it is impaired glucose tolerance. Both these conditions are also referred as pre diabetic stage. Normal glucose is - fasting 70 to 110 mg and pp <140 mg.
