Laser Prostate Surgery And Its Procedure!

One of the most common problems faced by ageing men is benign prostatic hypertrophy. Prostate is the gland at the base of the urethra near the bladder and when it enlarges it can lead to symptoms, mostly related to urination.

1. Frequent urge to pass urine
2. Prolonged urination
3. Frequent nocturnal visits to the toilet
4. Intermittent urination
5. Difficulty to start urinating
6. Inability to completely empty the bladder
7. Urinary tract infections

There are medications available to manage this, but offer only temporary relief. Many men therefore prefer to have the surgery undergone to manage these bothersome symptoms. However, like any surgery, the risks and benefits need to be considered along with other conditions like age, overall health status, other comorbid conditions, etc.

Surgical removal of the enlarged prostate gland is a more definitive approach to manage these symptoms. In addition to providing a quick cure, it also is used in the following cases:

1. Patients who do not respond to medications
2. Presence of blood in the urine
3. Associated bladder stones
4. Frequent infections of the urinary tract
5. Associated damage to the kidneys

Procedure of Surgery
During the procedure, a tube is passed through the tip of the penis into the urethra towards the bladder neck. Once it is in the desired position, laser is passed through it to deliver energy that acts on the prostate to either completely or partially destroy it. There are two methods by which laser acts on the enlarged prostate and making way for free flow of the urine.

1. Ablation: Excess prostate tissue is melted away by the laser by using photosensitive vaporization of the prostate. This is also known as Greenlight laser therapy or KTP laser vaporization. Alternately, Holmium can be used as the source of laser energy to ablate the prostate tissue.

2. Enucleation: Excess prostate tissue is cut and teased out through the urethra. Holmium laser is used to resect the prostate into smaller pieces, which are then removed out through a resectoscope. Another technique uses a tissue morcellator which grinds the enlarged prostate into smaller pieces to enable easy retrieval.

More men now opt for laser prostate removal as it has the following advantages:

1. Reduced risk of bleeding: This becomes essentially important in patients who are on blood thinners.
3. Immediate symptom relief: As compared to medications, the relief is felt almost immediately after the surgery
4. No catheter: With laser surgery, a catheter may be required for less than 24 hours unlike in open surgical cases.

As noted above, as with any surgery, once enlarged prostate symptoms set in, have a detailed discussion with your doctor to identify if you are a suitable candidate for laser surgery. If you wish to discuss about any specific problem, you can consult a Urologist.

Basic symtoms of breast cancer are 1 painless lump inbreast 2 lump in axilla 3 prevention is by early detection, investigations ultra sound and mammography and f. N. A. C or biopsy. If results are positive, management should be done.

Its not cancer for sure. Cancer will never cause pain. Its probably related to your age - either because 1] your breasts are enlarging/ growing or 2] related to your menses or 3] Costochondritis - which means a peculiar condition affecting the joint of the ribs under your chest Kindly give more information regarding your menstrual cycle and if the pain is cyclical or non cyclical.

We need to do proper diagnostic tests before concluding anything. Recurrent ulcers are related to systemic problems or can be due to regular medications kindly consult a dentist as well as physician to rule out the cause. Latest treatment what we can do is laser treatment for ulcer.

Hi Mr. Lybrate-user, I appreciate you seeking help. Well, the biopsy report is indicative of invasion to the lamina propria. But, can you doubly check with your doc if there is any invasion into the lymphatics too? so far as I know, such cases are treated with bcg vaccination, assuming there is no lymph node invasion. Additionally, given to understand that its a high grade carcinoma, there always is a high risk of metastasis despite a tur-bt surgery. Hence, the need to aggressively manage it, to the extent of even going for a full bladder cystectomy should it so demand, to help avert a futuristic metastasis and improve prognosis. Anyways, do clarify the above-mentioned through an expert second opinion with an oncologist specializing in urology cancers. From a dietary standpoint, try including more of fresh turmeric, carrots, tomatoes, cabbage, broccoli, green tea (twice daily) etc all besides other leafy greens. Pomegranate juice & vitamin c (can try'amalaki') helps too in this condition. Hope this helps. Do take care and all the very best to you.

Age 56. How to prevent prostrate from enlarging? psa count normal. Urine flow test normal so far. Ultrasound indicates enlarged prostrate. Is there a preventive method to stop prostrate enlargement?

There are medicines eg tamsulosin, alfuzocin etc to prevent the further enlargement and studies indicates about 20 to 25% reduction in size of prostate.

Hello doctors, my mother had breast cancer operation 3 week ago. Now the doctors are telling me to have chemotherapy. But I don't want her to have chemotherapy and I requested them to give her oral chemo but they are not at all ready to do so? So will oral chemo work or not and is it necessary for chemotherapy. As the doctors have said that they have no guarantee that even after giving chemo the cancer will not be created again . So refer me !

Dear sir, post breast surgery, depending on histopathology reports, size and hormone receptor status, chemotherapy is decided. For breast no oral chemotherapy is done, however if she is hormone receptor positive, hormonal therapy may be done as a systemic treatment, but detailed histopathology reports are required to decide about further treatment, so please share.

Nevertheless, to be on a safer side, I would advise you to get this clinically examined by a gynaec oncologist right away and not delay any further. Please note that imaging, either through mammography or mri, followed subsequently by a biopsy is always the gold standard for accurate diagnosis of breast cancer. Hi protyasha, seems like you have contracted some sort of food poisoning. The pain is because of vomiting. You can take tablet domstal twice a day and along with it take tablet meftal spas. That should relieve you of your symptoms.

Hi lybrate-user, frequent urination, muscle weakness & cramps, whole body bone pain, high blood pressure, high blood sugar, weight gain, headache etc all are commonly seen. Above-mentioned apart, gynecomastia/ tenderness of breasts could also be seen in male members if there is excess of estrogen hormone produced in the body. This may also result in reduced sex drive/ impotency. On the other hand, an excess of androgen production could lead to excessive hair (body) growth, and also enlargement of the sex organs. Dear sir there no mathematical Co relation, but smokers are twenty times more at risk than non smokers and even passive smokers, those who live in company of smokers are at risk of getting cancer which may be about five times higher than completely nonsmokers.
