the idea that anti diabetic medications cause long term bad side effects is itself wrong. There millions of patients on oral anti diabetic drugs, properly controlling blood sugar levels and living a good life. To my knowledge there are no drugs in other branches of medicine which have any action on insulin secretion or reducing sugar levels. As regards reducing the fasting levels you may have to give a re look at your diet and total calories consumed in a day. Or you may have to add another drug to control sugar level.

Since your father has diabetes, it is advisable that you take care. Excessive sweets will make you put on weight and that is a well known risk factor for the development of diabetes.

I am 37 years old I have high level of uric acid in blood, I am having ankle pain what should I do?

MBBS, MD - Internal Medicine

I am 37 years old I have high level of uric acid in blood, I am having ankle pain what should I do?
A high uric acid level can be caused when your body either produces too much uric acid or your kidneys don't eliminate uric acid rapidly enough. A high uric acid level may cause increasingly frequent attacks of gout, or it may never cause problems. A high uric acid level may also cause some people to develop kidney stones or kidney failure. And some people with a high uric acid level also develop high blood pressure, heart disease or chronic kidney disease, but it's often unclear whether this is a direct cause or merely an early warning sign of these conditions. Factors that may cause a high uric acid level in your blood include: Diuretic medications (water pills),Drinking too much alcohol,Genetics (inherited tendencies),hypothyroidism (underactive thyroid),Immune-suppressing drugs,Niacin, or vitamin B-3, obesity, psoriasis, Purine-rich diet — organ meat, game meat, anchovies, herring, gravy, dried beans, driedpeas, mushrooms and other foods Renal insufficiency — inability of the kidneys to filter waste Tumor lysis syndrome — a rapid release of cells into the blood caused by certain cancers or by chemotherapy for those cancers,Also, you may be monitored for high uric acid levels when undergoing chemotherapy or radiation treatment for cancer. Avoid protein diets, avoid more milk products. bad habits , fast food/fat/oily,salty foods. Coffee, smoking, alcohol intake. Non-veg., take whole grain, oats, green veg.,plenty of water in day time, 1 glass of water at night before bed. Take tab. Zyloric 100 mg once a day. Take tab. Bencid 500 mg once a day. Tab. Ulracet sos for pain in joints. Doctor may be give corticosteroid oral or inject direct in to joint.

Its sad that despite being on an insulin pump still sugars are uncontrolled. The best treatment modality for a type 1 dm is insulin pump. I suppose you or your daughter is not using it properly. You are supposed to adjust /titrate bolus insulin before every meal depending on heavy the meal is or its average calorie content. Then basal insulin also has to be adjusted. You can defenitely get good glycemic control with this.

Doctor I am a diabetic patient. My sugar level goes always high so what should I do to control it.

Doctor I am a diabetic patient. My sugar level goes always high so what should I do to control it.
Increase thirst, increased frequency of urination are some initial symptoms of diabetes but it can vary from being completely asymptomatic to non specific symptoms like weakness, tingling in feet, dry mouth, blurry vision, burning during urination. You need to follow a healthy lifestyle to prevent such metabolic diseases. Type 1 usually occurs at younger age due to insulin deficiency and treated with insulin injections. Type 2 usually in adults due to insulin resistance and treated with oral medicines.

Hypothyroid leads to a slight increase in Triglycerides, your levels are not high enough to start any medications yet. you have not mentioned your LDL levels. so make sure your TSH is within normal range. Secondly diet, exercise and lifestyle patters have a major correlation with Cholesterol levels, It is important that you modify your diet to a low fat, low sugar diet (take a detailed diet consult for that) and begin with exercise, start gradually increase to 45 mins per dayof brisk walking or yoga, quit alcohol if u take. these are standard recommendations asper American and European guidelines accepted by India cardiac society also, so kindly do not pay heed to anyone who suggests to you that diet has no role to play in cholesterol management.

How To Deal With Erectile Dysfunction If You Are A Diabetic?

PhD SEXUAL MEDICINE, DGO, MBBS
Sexologist, Ahmedabad
How To Deal With Erectile Dysfunction If You Are A Diabetic?

Erectile dysfunction is a disorder where the person affected is unable to maintain or achieve an erection during sexual intercourse. As diabetes is a disorder that affects blood supply to organs, so the ability to achieve or maintain erections are impaired, as erections are dependent on proper blood supply to the penis. Fortunately, there are treatments for these symptoms and they are:

  1. Injection therapy and oral medications: Some medications are directly injected to the penis in order to facilitate erection, while certain other oral drugs are administered to treat symptoms of erectile dysfunction which have a higher success rate in treating people who are diabetic than those who are not.
  2. Regular exercise and a healthy diet: Exercising on a regular basis combined with a healthy diet can reduce the symptoms of both diabetes as well as erectile dysfunction.
  3. Reviewing medications: Some medications used to treat blood pressure and depression can often cause erectile dysfunction. So, the medications should be reviewed by a doctor and, if necessary, should be changed.
  4. Usage of vacuum constriction devices: These devices are used to create a pump in the penis which facilitate the ability to maintain an erection and engage in sexual intercourse. It is reported to work in 80% of the cases.
  5. Penile implants: It is a surgical procedure where a penis implant is used to treat this disorder.
  6. Intraurethral therapy: In this treatment, the medicine is placed in the urethra of the penis in the form of suppository by an applicator. These medicines improve blood flow and thus facilitate a normal erection.
