My openion is to keep TSH below 3 for which you should take thyroid hormone because it is also required for mental and physical development of foetus. Consult your obstetrician also.

I am a 40 years old diabetic patient last 9 years and my latest fb is 146, pp is 233, Hba1c is 6.5,creatinine level is 1.4,SGPT is 65, creatinine spot urine is 319.87,mitral urine-40.9,TSH-5.52,Weight-55.70,urea-74, cholesterol- 171, Triglycerides-139, SGOT-35, LDL Cholesterol- 43, LDL cholesterol-108, WBC-13.8,Haemoglobin-15.1,fatty liver grade 1, bp-150/90. Is it any serious in my report now? Please tell me how to control all of these?

Mr. lybrate-user Dey, From the details given these are some conclusions: 1) Fasting and PP glucose levels are high. Idealy FBG should be closer to 100 mg, PP 150 to 160 mg. HbA1c% is good. It means you have to be careful in your daily food intake. 2 )TSH is normal. 3) Rest of the parameters (LFT, lipids & blood picture) are normal. 4) Grade fatty liver is not very significant. But if you indulge in alcohol then stopping it is ideal. 5) BP is high, it should be 130/80 mm Hg. Thanks.

Hi, Type-2 Diabetes is a lifestyle related problem, so making a few lifestyle changes can lower the chances of developing type 2 diabetes. -Excess weight is the single most important cause of type 2 diabetes, so control or reduce your weight if you are overweight! -Avoid sedentary lifestyle and start get moving. Brisk walking for a half hour every day reduces the risk of developing type 2 diabetes. -Modify your diet: *Choose whole grains and whole grain products over highly processed carbohydrates. The bran and fiber in whole grains make it more difficult for digestive enzymes to break down the starches into glucose. *Avoid sugary beverages which have a high glycemic load, and drinking more of this sugary stuff is associated with increased risk of diabetes. *Choose good fats instead of bad fats. Good fats found in liquid vegetable oils, nuts, and seeds. Trans fats or bad fats are found in many margarines, packaged baked goods, fried foods, fast or junk foods. *Limit intake of red meat and processed meats. Nuts, seeds, beans, tofu, or poultry are much more healthful protein choices. -Quit smoking. -Moderate alcohol consumption decreases the risk of type 2 diabetes. -Avoid taking unnecessary stress or tension! And keep an optimistic attitude in life! Medication: Take homoeopathic SBL’s Diaboherb medicine/ 3 times a day daily. Home Remedies: Dry the leaves of mango tree and grind to a powder. Mix 1 teaspoon dry powder in a glass of water and drink it daily to reduce high blood sugar levels. Take 2 teaspoons of bitter gourd (karela) juice once a day. One can also increase its use as a cooked vegetable. Take 1 teaspoon of Indian gooseberry (amla) juice mixed with 1 teaspoon of bitter gourd juice twice a day.

plug your ears with cotton while taking bath.Do not try to clean your ears yourself. Continue medication as prescribed till ear is dry and no discharge is present. change medication In case you are not responding.

Infertility - Are You at Risk?

Infertility is a condition where a couple is trying to conceive in a natural way and is not able to do so even after one year of trying. It is also a matter of worry for patients who are over the age of 35 years and those who are not able to conceive naturally even after 12 months of trying constantly. So here are the causes, risks and treatment for this condition.

Causes: There are many causes of infertility. It may be down to a condition present in one or both partners. As per a medical study, 20% of the cases of infertility are due to males, while 40% to 50% cases are due to problems in the female partner. 30% of the cases suffer from infertility due to problems in both males and females. Ovulation and fertilisation are the main elements of the conception process, and any condition in the female or male can upset either function which can make it difficult to conceive. While abnormal sperm production and transport may be a common condition that leads to infertility in males, other factors like side effects of cancer treatments and too much exposure to toxins and chemicals can also play an important role in this regard. For females, the causes of infertility may range from ovulation disorders, uterine and fibroid tumours, uterine and cervical anomalies, damage to or blockage of the fallopian tubes, endometriosis, primary ovarian insufficiency and pelvic adhesions. Also, factors like thyroid, cancer and medication leading to temporary infertility may be at play. 

Risk: There are a number of factors that put a male or a female at risk of infertility. Being in your mid 30s usually has an adverse effect on the ovulation and production of well functioning sperm. Further, smoking and tobacco are among the main causes of infertility in either partner. Excessive alcohol consumption may also lead to this problem. Being overweight or underweight can also make a person infertile. Further, not getting enough exercise and routine activities out of your day, can lead to this condition as well. 

Treatment: Male patients can go through treatment for any existing infections, hormonal injections and medication, surgical options like vasectomy reversal, and ART or assistive reproductive technology. Female patients can turn to medication that will stimulate ovulation, Intrauterine Insemination, and surgical processes to correct a uterine septum and remove endometrial polyps, or even a hysteroscopy surgery. 

If these methods fail, the patient or the couple can always turn to methods like in-vitro fertilisation. For other mild reasons, lifestyle changes along with a stress free environment can help in increasing the chances of conception. If you wish to discuss any specific problem, you can consult a gynaecologist.

Hello, my father is hypothyroid past 12 yrs, his TSH levels are round 12-14 mIU/ ml and used 100 mcg thyroxine tablets .he stopped his medication for around 20 Days recently and now his TSH is 55 .how much dosage of thyroxine should be increased so that he can use 100 mcg again?

Hello, Thanks for the query. For a person with hypothyroidism, treatment has to be continued without any break. Stopping of thyroxine will result in increase of TSH and reappearance of all the unwanted symptoms. Ideally TSH should be maintained around 3 to4 mU/L. But in older patients it can be around 5 mU/L. It is difficult to say whether TSH level will return to above mentioned by restarting 100 mcg tablets. But it is better to start that and then check TSH after 6 weeks. Thanks.

Nothing to be done now. Atleast till you get married and want to conceive. If overweight loose some weight. Get your bmi in normal range.
